About this file

This document is a Cost Proposal Form for the Provo City School District's Mathematics Curriculum Adoption, spanning elementary (K-6), middle (7-8), and high school (9-12) levels. The district serves approximately 6,600 elementary students, 2,000 middle school students, and 4,600 high school students, with 255 elementary teachers, 20 middle school teachers, and 35 high school teachers. The solicitation seeks comprehensive mathematics curricula for all grade levels across 13 elementary schools, 2 middle schools, and 3 high schools. The contract term is seven years with a potential 1-3 year renewal, and the response deadline is Friday, October 17th, 2025 at 2:00 PM Mountain Time.

The Cost Proposal Form requires vendors to provide detailed pricing for student licenses, site/school licenses, and teacher licenses, with a total seven-year cost breakdown. The form requests pricing for shipping and handling, professional development, and invites vendors to list included materials and any volume-based discounts. The procurement is being conducted by the Provo City School District's Department of Purchasing and Department of Curriculum, with the intent to award a single contract to the most responsive and responsible vendor based on scoring criteria. All pricing must be guaranteed for the entire contract term, and student licenses must be transferable between grades to accommodate potential population fluctuations.
