About this file

This is a Minority/Woman-owned and Small Business Enterprise (M/W/SBE) Utilization Form required by the City for the Boiler, Gas System, Preventative Maintenance and Repair Services contract opportunity with Charlotte Water in North Carolina. The form mandates that companies submitting proposals identify their own M/W/SBE certification status (if applicable) along with the certifying agency and certification dates. Proposing firms must document all outreach efforts undertaken to identify and include M/W/SBE subcontractors both during the proposal phase and throughout the contract performance period. The form requires submission of a comprehensive subcontractor list detailing each M/W/SBE partner, their description of work or materials, relevant designations, city vendor numbers, and calculated utilization percentages for minority-owned, woman-owned, and small business enterprises.

The City of Charlotte maintains a strong commitment to M/W/SBE inclusion in its contracting processes and actively encourages companies to exhaust all reasonable efforts in identifying eligible subcontractors for participation in this RFP. Proposing companies must demonstrate their outreach strategy both retrospectively (efforts already undertaken) and prospectively (efforts to be employed during contract performance). The form requires signature and dating by an authorized company representative and must be submitted with the proposal package.
