About this file

This is an Identification of Subcontractor/Supplier/Subconsultant Form (ISF) issued by the Cook County Office of the Chief Procurement Officer in Illinois for use with bids, proposals, and qualifications. The form requires prime contractors to identify and fully disclose all subcontractors, suppliers, and subconsultants that will be utilized on awarded contracts. Contractors must submit a completed ISF with each bid or proposal and are obligated to file updated forms if any changes occur in subcontractor or supplier utilization during contract performance. The form applies to the Heart Rate Monitoring System procurement (RFP 2517-02130) and requires detailed information for each subcontractor including authorized contacts, company addresses, telephone and fax numbers, estimated start and completion dates, and a description of services or supplies with associated subcontract pricing.

The ISF mandates that all subcontract documents incorporate the requirements of the prime contract and that subcontractors must not allow work on the contract to hinder their progress on other concurrent engagements. Prime contractors retain full responsibility for organization, performance, and quality of work regardless of subcontractor utilization. The form explicitly states that ISF submission does not approve any changes to the contract's approved Minority Business Enterprise (MBE) and Women Business Enterprise (WBE) Utilization Plan, and any modifications to the approved plan must be submitted separately to the Office of Contract Compliance. Upon request, prime contractors must provide copies of all written subcontractor agreements to the Cook County Office of the Chief Procurement Officer.
