About this file
This is an RFP evaluation scoresheet for the State of Utah Division of Purchasing and the Department of Health and Human Services for procurement MK26-23, Pharmaceutical Blister Pack and Label Supply. The RFP seeks to procure blister cards, sealing paddles, and pharmacy labels for the DHHS Pharmacy, with an anticipated single contract award to the highest-scoring responsive and responsible vendor. The contract term is five years, with vendors required to guarantee pricing for a minimum of one year.
The scoresheet establishes evaluation criteria weighted as follows: technical evaluation comprises 70 percent of the total score (1,000 points), with a minimum required threshold of 700 points, and cost evaluation comprises 30 percent (428.6 points). Technical scoring is based on five primary criteria: detailed overview of DOSIS cold-sealed and heat-sealed blister pack offerings (250 points), capacity for high-frequency, low-volume distribution (100 points), quality assurance processes for prescription label alignment and printer compatibility (250 points), heat-sealing equipment integration and downtime mitigation strategy (200 points), and overall ability to meet DHH operational needs including supply reliability and workflow compatibility (200 points). Six mandatory requirements must be satisfied on a pass/fail basis: ability to supply heat-sealed blister packs with large and extra-large blisters, DOSIS cold-sealed blister packs and lids, prescription labels for DOSIS tower, perforated prescription labels for Zebra printers, heat sealers and compatible paddles, and maintenance of all necessary licenses. Cost points are assigned using a formula whereby the lowest-priced offeror receives 100 percent of available cost points, with all other offerors receiving proportional points based on their price relative to the lowest proposal.

| * Purchasing will use the following cost formula: The points assigned to each offerors cost proposal will be based on the lowest proposal price. The offeror with the lowest Proposed Price will receive 100% of the price points. All other offerors will receive a portion of the total cost points based on what percentage higher their Proposed Price is than the Lowest Proposed Price. The formula to compute the points is: Cost Points x (Lowest Proposed Price/Proposed Price). | ||||||||||
