About this file
This is a Mandatory Minimum Requirements and Technical Scoring Criteria attachment for RFP MK26-23, issued by the State of Utah Division of Purchasing on behalf of the Department of Health and Human Services. The procurement seeks a single vendor to supply pharmaceutical blister packs (both heat-sealed and cold-sealed DOSIS-compatible formats with large/extra-large 30/32 blisters per card), prescription labels compatible with DOSIS towers and Zebra printers, heat sealers, paddles, and related equipment for the DHHS Pharmacy. The contract will be awarded to the responsive and responsible vendor with the highest combined score and has an anticipated term of five years.
Vendors must meet all six mandatory minimum requirements to advance to the technical evaluation phase, which includes five scored criteria evaluated on a 0-5 scale. The mandatory requirements include the ability to supply specified blister pack formats, DOSIS-compatible cold-sealed packs with lids, prescription labels meeting current requirements, perforated labels for Zebra printers, heat sealing equipment compatible with the packs, and maintenance of all necessary licenses. Vendors must also demonstrate technical competency in DOSIS offerings, capacity for high-frequency low-volume distribution, quality assurance processes ensuring 100% label alignment with CHS templates, heat-sealing equipment integration strategies, and overall ability to meet DHHS operational and supply reliability needs. Vendor pricing must be guaranteed for one year from the time of submission.
