About this file
This is a Scope of Work document for the State of Utah Department of Health and Human Services (DHHS) Division of Purchasing, establishing requirements for pharmaceutical blister pack and label supply services to support medication packaging and dispensing operations across Utah State Correctional Facilities. The contract calls for the supply of specific blister cards in multiple sizes and dose configurations (30-dose medium, large, and x-large cards at 500 per case, and 60-dose cards at 250 per case), sealing paddles in two units, prescription labels in both perforated and non-perforated formats, and a desktop heat sealer capable of accommodating dual cards. All products must be compatible with the DHHS Pharmacy's DOSIS automated blister-packaging machines and meet specified equipment requirements. The contractor is responsible for ensuring shipping, handling, and delivery are included in product pricing, with delivery to 1480 North 8000 West, Salt Lake City, UT 84116-3961. The contract term is five years with vendor pricing guaranteed for one year, and the procurement is anticipated to result in a single contract award to the highest-scoring responsive and responsible vendor.
Pricing is to be provided on a per-unit or per-case basis for each product line item as specified in the pricing table, with additional pharmaceutical supplies, components, or labels available for written approval at the time of request. The contractor must maintain all necessary licenses and submit employee information including names, dates of birth, driver's license numbers, and social security numbers for background screening, with ongoing submissions required for any new staff providing on-site services. Billing shall occur monthly with invoices detailing products provided, quantities, and pricing aligned to the contract schedule. Performance metrics include the percentage of supplies meeting DHHS specifications, percent of orders fulfilled within required response times, number of supply shortages or delivery delays, and corrective action resolution timelines. Total contract funding is based on average usage projections, with provisions for funding increases if usage exceeds anticipated levels, subject to DHHS approval and contract amendment.
