About this file

This is a General Data Protection Regulation (GDPR) compliance questionnaire from the University of Texas Health Science Center at San Antonio (UTHSCSA) related to a Graduate School of Biomedical Sciences Customer Applicant System (CAS) procurement. The questionnaire is designed to ensure that any department or program storing, processing, or utilizing personal data of individuals located in the European Union complies with GDPR requirements. The form requests comprehensive information about the scope of personally identifiable information maintained, including whether the organization stores data on EU residents or individuals temporarily located in the EU, the types of personal data collected by activity, methods of data collection, consent procedures, and data retention practices. Additionally, the questionnaire addresses website privacy policies, visitor notifications, consent mechanisms for data collection, and the handling of sensitive data categories such as racial or ethnic origin, health data, genetic or biometric data, and criminal convictions.

The Graduate School seeks a vendor to manage its centralized admissions application system for 18 academic programs, 6 summer programs, 1 postdoctoral program, and 1 high school program. The requested solution must integrate with the University's PeopleSoft and the school's Nutrient software systems, enable faculty review capabilities, and provide robust Customer Relationship Management (CRM) functionality for prospect communication, tracking, and admission offer delivery. The current application vendor has significant integration limitations and insufficient CRM capabilities, requiring the admissions office to utilize a separate vendor for CRM functions. No guaranteed volume, compensation, or minimum or maximum service amounts are specified. The questionnaire does not include specific pricing terms, set-aside provisions, response dates, contract term details, or renewal options.

The General Data Protection Regulation (GDPR) is a regulation that protects the personal data and privacy of individuals in the European Union (EU). This questionnaire must be completed by departments or programs that are storing, processing, or otherwise utilizing any personal data of individuals in the EU to ensure that current processes comply with GDPR.

Compliance with GDPR requirements is triggered for individuals when they are located in the EU and is not required for EU citizens/residents when they are located in the United States. Additionally, the regulation can be applicable to a U.S. citizen located in an EU country; residency or citizenship in an EU country is not required for applicability of the regulation.
