About this file

This is a Limited Asbestos Inspection and Lead-Based Paint Survey report prepared by Hill West Environmental for the State of Utah Department of Administrative Services, Division of Facilities Construction and Management (DFCM), regarding the Utah State Hospital Warehouse Building located at 1300 East Center Street, Provo, Utah. The inspection was conducted on July 21, 2026, to identify asbestos-containing materials (ACM) and lead-based paint within areas scheduled for renovation, specifically the Fire Alarm System upgrade and Hydronic Heater upgrade projects. The survey covered suspect materials including sheetrock wall systems and 2x4 ceiling tiles, with six asbestos samples collected and analyzed using polarized light microscopy (PLM) and lead content measurements obtained using Niton XL2 X-ray Fluorescence (XRF) analysis with a detection limit of 0.1 to 0.3 mg/cm². The inspection was limited in scope to only areas impacted by the planned renovation activities. The report was prepared by Claude Dahlk, CHMM, CIEC, an asbestos inspector certified under ASB-0433, and submitted to Ms. Aubrey Stapel, Project Manager with DFCM, on August 12, 2026.

The survey results indicate that no asbestos-containing materials were detected in any of the six samples analyzed, with all samples showing "ND" (no fibers detected) results. Similarly, no lead-based paint was detected within the building components tested, with all XRF readings below the 0.4 mg/cm² threshold. The report notes that universal, hazardous, and toxic wastes will not be impacted during the renovation project. The survey was conducted in compliance with EPA regulations as administered by the Utah Division of Air Quality, UOSHA federal OSHA regulations, and Salt Lake Valley Health Department (SLVHD) demolition regulations. The analytical results were certified by Eurofins Aerotech Built Environment Testing, LLC (NVLAP Lab Code 500031-0, EMLab ID: 4663921), with analysis completed on August 12, 2026. The report specifies that if contractors discover additional suspect materials not sampled during this inspection, they must cease work and notify DFCM for additional sampling before proceeding.

1.0 INTRODUCTION

As per DFCM’s instructions, Hill West Environmental conducted a limited asbestos and lead-based paint inspection of the Utah State Hospital - Warehouse Building located at 1300 East Center Street, Provo, Utah 84606.

Due to the remodel and upgrades to the Fire Alarm System and Hydronic Heater, Hill West was instructed to only inspect areas that will be impacted during the renovation project.

The purpose of this survey was to identify the existence, extent, and condition of both friable and non-friable asbestos-containing materials (ACM) and lead-based paint/lead materials within the renovation areas.

2.0 SITE VISIT /OBSERVATIONS

2.1 Asbestos Inspection

On July 21, 2026, Hill West Environmental visited the site to survey the renovation areas of the building. A complete asbestos inspection of the areas that will be impacted during renovations was made to determine conditions and develop a sampling scheme for the areas. The materials that will be impacted during this upgrade project include sheetrock wall system and floor tile. Table 1 summarizes the asbestos inspection results.

2.2 Lead-based Paint Inspection

On July 21, 2026, Hill West Environmental conducted a limited lead-based paint inspection of the Utah State Hospital – Warehouse Building within the project work area. Direct measurement of lead in paint/dust were made using a Niton XL2 X-ray Fluorescence (XRF) Spectrum Analyzer. The Niton XRF non-destructively measures lead concentrations of painted surfaces, regardless of the number of layers present.

According to the manufacturer, the detection limit (lower limit of reliable measurement) for this instrument is 0.1 milligrams per square centimeter (mg/cm2) ±0.3 mg/cm2 with the instrument set on the “quick” measuring mode.

The quick mode provides 95% confidence that the lead concentration in the paint is above or below the set point of the instrument which, for this survey is 0.4 mg/cm2. For this survey, measurements below 0.4 mg/cm2 were not reported as lead containing.

The Niton XRF sometimes reports negative values. According to the manufacturer, negative values should be expected and interpreted as zero lead content due to the statistical variability of XRF measurement technology. Both HUD and the EPA recognize the statistical variability of XRF technology and the possibility of obtaining negative values where the lead content is near zero.

Table 2 summarizes the lead-based paint inspection results. Measurements are reported in milligrams of lead per square centimeter (mg/cm2). All the coatings tested during this inspection were in good to fair condition. Only positive readings (>0.4 mg/cm2) are summarized in Table 2. Appendix B contains all XRF readings for this survey.

4.0 DISCUSSION/CONCLUSIONS

In Utah, EPA asbestos regulations are administered by the Utah Division of Air Quality. Utah Occupational Safety and Health Administration (UOSHA) has adopted the Federal OSHA regulations. In addition, the Salt Lake Valley Health Department (SLVHD) regulates demolition activities in Salt Lake County. The SLVHD regulations for pre-demolition building inspections require an asbestos inspection, but also required building owners to inspect the building for other hazardous materials such as universal wastes, hazardous and toxic wastes and lead-based paint. Similar to asbestos, these wastes, if present must be removed prior to demolition/renovation.

A copy of this report should be on-site during all renovation activities.

This limited asbestos and lead-based paint inspection was conducted on the project area that will be impacted during renovation activities. Only items that are believed to be impacted during renovation activities were inspected during this survey. This survey is not intended to represent an exhaustive research of all asbestos or other hazards or conditions which may exist within other buildings located at this project site.

This report does not purport to represent future indoor conditions or events.

Situations or activities which transpire subsequent to this report, which result in adverse environmental, construction and/or engineering impacts, are not to be construed as relevant to this study.
