Bidders must also verify they are not City officials or employees and disclose any outstanding claims against their firm. The document requires CCB (Contractor's License Board) registration information and details of surety company contacts for the Payment and Performance Bond. Bidders must certify that their submission is genuine and not collusive, and that they will comply with all applicable City, State, and Federal regulations.
