About this file

This document is a Foreign Entity Ownership Affidavit required by Florida Statutes for bid submissions or contracts that provide an entity access to personal identifying information. The affidavit is a mandatory form for entities participating in state and local contracts, designed to ensure that bidding organizations are not owned, controlled, or principally located in foreign countries of concern as defined in Sections 287.138, 692.201, and related Florida Statutes.

The affidavit requires the entity to attest to nine specific statements, including not being owned by a foreign government, not having a controlling interest from a foreign country of concern, and being in compliance with Florida Statutes Sections 692.202, 692.203, and 692.204. The document must be signed by an authorized representative of the entity, with spaces provided for signature, printed name, title, entity name, and date. This form is particularly relevant for entities seeking to bid on contracts or purchase real property in Florida, ensuring transparency and security in state-level business transactions.
