STATEMENT OF WORK

OR FLAP DOUGLAS 251(2) Winchester Bridge Replacement Geotechnical Investigation

Douglas County, Oregon

I. INTRODUCTION

The Western Federal Lands Highway Division (WFLHD) of the Federal Highway Administration

(FHWA) is preparing a geotechnical subsurface exploration program for the Winchester Creek

Bridge in Douglas County, Oregon. The primary goal is to assess the subsurface conditions for designing a replacement bridge structure. Agency partners include the U.S. Forest Service.

The project is located at latitude N 43.6751° and longitude W 124.1788° in the northeast ¼ of the southwest ¼ of Section 7, Township 22 South, Range 12 West of the Bureau of Land Management

(BLM) Public Land Survey System (PLSS). Project impacts are limited to the existing roadway.

The project location is shown on

Attachment 001 – Boring Location Map.

The boring locations are underlain by embankment fill materials, alluvial deposits, estuarine deposits, and the Tyee Formation. The areas extending east and west of the existing abutments maintain an elevation of approximately 11 feet. The two borings will be advanced; one near the existing west bridge abutment (boring BH26-01), and one near the existing east bridge abutment

(boring BH26-02).

This requirement includes the following:

• Drilling 2 geotechnical borings through the existing bridge approaches.

• P-S suspension logging in the upper 100-ft of one boring.

• Traffic control, asphalt coring, borehole completions in accordance with State of Oregon requirements, and asphalt patching.

Boring depths will range from 50 to 130 feet below the existing ground surface (bgs). We anticipate the drilling methods will include hollow-stem auger (HSA) or mud-rotary drilling in the overburden materials transitioning to HQ3 coring in the underlying bedrock.

Temporary traffic control is required for this work. Submit a traffic control plan prior to start of work to allow for a single lane closure. The closure will be in place during daylight and operational hours and removed each night.

The Contractor is responsible for fully implementing all environmental commitments identified in this Statement of Work (SOW) and attachments.

The planned start date is late-November to early-December for all activities, and flexible depending on the selected contractor availability. We anticipate all work should be completed within 5 business days.

B-2

II. STATEMENT OF WORK FOR DRILLING (BASE)

The Federal Highway Administration (FHWA) Contracting Officer (CO) is the only representative that has the authority to revise or add work under the requirements of this solicitation Onsite representation for FHWA will be provided by the Contracting Officer Representative (COR).

The test borings coordinates for the investigation contained within this SOW are shown in

Attachment 002 – Table of Borings. Boring locations will be marked in the field by FHWA with white paint, wood staking, or some other physical mark in time to call in utility locates. The integrity of the boring location markings will be verified no later than during the on-site Pre-Drill

Meeting with the Contractor and subordinate parties (See Section I, Pre-Drill Meeting). The Table of Borings provides detailed information, including the boring location with GPS coordinates

(WGS84), estimated depths for each drilling method, maximum drilling depth, and expected numbers of in-situ tests. Access notes for the boring locations are also included.

A. Drill Type and Tooling

Provide the necessary drill rig(s) to complete the work and provide additional drill rigs as necessary to perform the specified work. WFLHD’s interpretation of the most practical means to access and advance the test boring for this project is using a truck mounted drill rig. However, the selection of the drill rig type is to be determined by the Contractor. Tracked or other equipment that leaves indentations in pavement is not allowed to track directly on the road without using protective mats.

Supply all necessary equipment to drill the proposed test borings, including:

• HSA drilling (4.25-inch ID), casing advancer, or mud rotary (3 7/8-inch OD)

• HQ3 wireline core drilling system (must be triple barrel coring)

• Standard SPT 2-inch OD, and oversized 2.5-inch O.D. or 3-inch O.D. split spoon samplers when coarse material is encountered, and

• A vacuum truck

Use a vacuum truck to clear the upper 5 feet of soils in all proposed borings. The COR will ensure no utilities are present in the upper 5 feet before continuing with the exploration. Advance the geotechnical borings using HSA drilling or casing advancer methods in the overburden soils and

HQ3 coring methods in the underlying bedrock to the drill depths indicated by the COR. Other drilling methods using drilling fluids, such as mud-rotary or casing advancer, may be used after groundwater is encountered. If heave or disturbance of the soil occurs due to hydrostatic pressures below groundwater level, the driller must flood the hollow center of the augers with water or an approved drilling fluid to maintain an adequate head of water so that drilling may continue.

Alternatively, or if adding water does not sufficiently limit heave, the driller should use mud rotary wash drilling techniques. HQ3 wireline coring is required in the borehole when bedrock is encountered. Provide waxed or plastic rock core boxes.

Supply a table, awning, and all required equipment for drill inspection and test boring logging.

B-3

Supply environmentally safe drill fluid additive or biodegradable lost circulation material (LCM) to be used in the event coring water does not circulate or drilling/coring fluids leak into the river.

B. Sampling Intervals

Drill and advance the borings in accordance with this SOW and the drilling methods listed above.

When in soils, collect SPT samples every 2.5 feet starting at 5 feet bgs to 20 feet bgs, and then every 5 feet until the maximum number of SPT samples listed in Attachment 002 Table of Borings or until transitioning to HQ3 core methods as directed by the COR. When advancing drill tooling in rock and the subsurface material is characterized as highly weathered, very weak to weak rock, take SPT samples between core runs to gauge the relative density of the rock material.

C. Driven Samples (SPT)

Collect driven samples using the standard SPT 2-inch split spoon sampler unless otherwise directed by the COR. Alternate use of the 2.5-inch and/or 3-inch diameter split barrel samplers in gravels, cobbles and boulder materials or where sample recovery with the standard SPT sampler is poor.

The Contractor must provide sampling tooling including split barrel samplers with a ball check and vent, and metal or plastic sample catchers in the barrel shoes. Replace or repair the driving shoe on the split barrel samplers if it becomes dented or distorted or has excessive wear. Replace the sample catchers when they become worn, broken, or no longer function.

Do not exceed the quantity of SPT samples listed on the quote sheet, unless authorized by the

Contracting Officer through a contract modification. The ceiling quantities for the base and option items are as follows:

Base item ceiling quantity for driven SPT samples is 44 SPT samples.

D. Shelby Tube Samples

Collect Shelby tube samples as indicated by the Table of Borings when a soft, saturated silt or clay layer is encountered. Additional Shelby tube samples may also be required as directed by the by the COR. The total number of Shelby tube samples that are taken under this contract must not exceed the number of Shelby tube samples listed on the quote sheet, unless authorized by the contracting officer through a contract modification.

• Base item ceiling quantity: 4 Shelby Tube samples

E. Drill Rig Automatic Hammer Efficiency

Use a drill rig equipped with an automatic trip hammer to complete the borings. Prior to mobilization, provide the most recent hammer efficiency reports for the drill rig. Reports that are older than 1 year at the time of contract execution will not be accepted. If reports are older than 1

B-4 year, have the drill rigs tested and submit the efficiency reports prior to the end of this contract performance period.

F. Drill Depths

Drill the borehole to the depths listed on the Table of Borings, or as directed by the by the COR.

The combined total boring footage should be exceeded with prior approval from the CO. Top of bedrock could be as shallow as 20 feet or as deep as 100 feet bgs. Recover cored bedrock samples to a depth of 20 feet below the elevation of when competent bedrock was first encountered, or to a minimum depth of 50 feet bgs. When doing P-S suspension logging, drill the hole to 130 feet to allow proper space for the suspension logging instrument.

G. Shared Ceiling Price for Drilling Methods

The following line items on the quote sheet have a shared ceiling price equal to the sum of the subtotals of all line items below:

• HSA drilling (4.25-inch ID);

• Casing advancer

• Mud rotary wash (3 7/8-inch OD); and

• HQ3 core drilling.

The Government will not be obligated to pay the Contractor any amount more than the ceiling price, and the Contractor shall not be obligated to continue performance if to do so would exceed the ceiling price, unless and until the Contracting Officer modifies the contract to revise the ceiling price.

The depths for each type of drilling method may exceed the estimated quantity shown in the quote sheet, provided that the shared ceiling price is not exceeded.

H. Suspension Logging

Perform P-S suspension logging in one of the borings (likely BH26-01), to be confirmed based on drilling conditions. Measure the Vs (shear wave) and Vp (compression wave) velocities at intervals/resolutions of 0.5 or 1 meter in the upper 100 feet (30 m) below the ground surface. Use a dual-receiver system to measure travel times between receivers, minimizing "borehole effects."

Drill the boring deeper to accommodate the suspension logger instrument, logging the boring and retrieving samples/cores throughout this depth (i.e., not a burn-down section). For initial planning purposes, we estimate that an additional 30 feet below the measured depth should be adequate (i.e., 130-ft total drill depth).

Coordinate drilling method, fluid type(s), and actual drill depths and diameter with the P-S suspension logging subcontractor, based on their requirements. Open-hole or uncased measurements are preferred. If casing is proposed or required due to subsurface conditions, submit

B-5 the casing material type, diameter, and grout specifications to the COR prior to performing P-S suspension logging testing.

Post-process the data and review waveforms for signal quality, filter noise, and pick first-arrival times for P- and S-waves. Compute Vp and Vs profiles as a function of depth. Calculate the harmonic mean shear wave velocity for the top 100 feet (Vs,30) using the formula:

Provide a technical report including:

• Introduction: Description of the site and methodology.

• Plots: Graphical plots of Vp and Vs versus depth.

• Tabulated Data: ASCII or Excel format of the velocity data.

• Summary: Calculated Vs,30 value and identification of the seismic site class. Discuss any anomalous data or issues encountered in the field.

• Calibration Records: Documentation confirming the equipment is within current service/calibration intervals.

I. Boring Location Access

The borings are located on the roadway approximately 10-feet from the edge of the bridge deck and can be accessed with a truck mounted rig to place the drill directly on the boring locations.

Construction of pioneer roads/trails will not be required.

J. Pre-Drill Meeting

Hold a pre-drill meeting onsite with the COR, WFLHD staff and Douglas County staff prior to start of drilling (same mobilization). At the pre-drill meeting the COR, Contractor, WFLHD staff, and Douglas County staff will review the boring locations and access with the Contractor. We anticipate this pre-drill meeting will require one (1) hour onsite, and may either be done the day before drilling commences or the first day of drilling.

During the pre-drill meeting the location of borings may be modified based on site conditions to minimize environmental impacts and/or to address utility conflicts and safety hazards that cannot be mitigated. Do not offset borings more than 5 feet from the staked location discussed during the

B-6 pre-drill meeting without prior approval from the COR. Coordinate the pre-drill meeting with the

COR at least 2 weeks prior to the meeting date.

The COR will provide a copy of environmental documents at the pre-drill meeting.

Notify the COR of any impacts to the public traffic a minimum of 7 days prior to start of impact.

K. Utility Locate

A public one call utility locate is necessary for this drill project. The boring locations will be marked in the field prior to that date (refer to Attachment 002 Table of Borings). The Contractor must complete the utility locate request a minimum of 72 hours prior to the pre-drill kickoff meeting and commencement of drilling.

• 1-800-342-1585

• 811 from anywhere in Oregon.

The Contractor will provide the utility ticket number(s) to the COR within 24 hours of completing the utility locate notification.

The Contractor will utilize a vacuum truck to clear the upper 5 feet of soils in each boring. The

COR will ensure no utilities which are not able to be located by the public one call utility locate are present in the borehole before continuing with the exploration.

L. Water for Drilling

Procurement of a water source for drilling is the responsibility of the Contractor and should be identified at least 14 days prior to the start of drilling. Water draws from the river must be permitted by the regional Oregon Watermaster, see website for contact information for District 15

.aspx). No water is to return to Winchester Creek.

M. Traffic Control

Traffic control is anticipated for this project. Provide a traffic control plan following the most current edition of the Manual on Uniform Traffic Control Devices (MUTCD) standards, as well as State and local requirements to safely allow for a single lane closure. Monitor the queues during operations for a maximum 20-minute hold delay. Road Access Permits

The borings are within Douglas County right of way and obtaining a Douglas County Public

Works (DCPW) Right of Way Permit will be the responsibility of the Contractor, including any associated fees. See website for more information on obtaining a DCPW Right of Way Permit

(https://www.douglascountyor.gov/368/Right-of-Way-Activity-Permits). WFL will provide pertinent information to the Contractor, as necessary, to complete the permit application.

B-7

N. Environmental Compliance Summary

The proposed exploration plan has been environmentally cleared by a Programmatic Agreement prepared by WFLHD.

The Contractor will follow the Geotechnical Activities Categorical Exclusion (Attachment 003) that include:

Follow all local and State regulations for backfilling a completed test boring, disposing of drill cuttings and containing and/or limiting any runoff from drilling fluids. Utilize best-management-practices for erosion control and use containers or tubs when recirculating drilling fluid is required.

• To minimize the amount of ground disturbance, locate staging and stockpiling areas in previously disturbed sites, including the roadway and gravel pullouts if available. Restore staging and stockpiling areas to pre-construction conditions following construction.

• Provide construction equipment and vehicles that are mechanically operating properly and do not excessively produce engine exhaust. Limit equipment idling to only what is necessary for safety and/or mechanical reasons. Equipment and vehicles will also be checked for leaking oil and fluids. Provide leak absorption kits on site.

• Provide and implement a construction spill prevention, control, and countermeasures plan.

• No unsecured fuel storage containers are allowed on the project site. Fuel will be delivered to the site only in trucks designed for fuel hauling, or in DOT approved fuel safety cans secured to vehicles.

• Construction workers and supervisors will be informed of the occurrence and status of special-status species (including federally listed species) and would be advised of the potential impacts on the species and potential penalties for taking or harming a special status species. Contract provisions would require the cessation of construction activities if a special-status species is discovered in the project action area. This would allow modification of the contract to include protection measures determined necessary to protect the discovery.

• The Contractor would be required to keep all waste and contaminants contained and removed daily from the work site. Food and other wildlife attractants would be contained to minimize risk of nest predators (i.e., corvids). Other mitigation measures to prevent human-wildlife conflict will include the following: feeding or approaching wildlife will be prohibited; a litter control program will be implemented during construction to eliminate the accumulation of trash; and all food items will be stored inside vehicles, trailers, or wildlife-resistant receptacles except during actual use to prevent attracting wildlife.

• Store chemicals in water-tight containers a minimum of 150 feet from waters and storm drain inlets. Keep containers closed, sealed, and secured when not being actively used. Use secondary containment for storage areas to capture leaks or spills.

• Conduct machinery maintenance and refueling 150 feet or greater from any wetlands or surface waters. Refueling of equipment may occur within 150 feet of waterbodies if spill prevention measures are in place and if approved by the COR.

• If active bird nests or bat colonies are identified during clearing operations, or if nesting, denning, hibernating or animals living within the worksite are identified, suspend work immediately and notify the COR within 1 hour and obtain approval before restarting.

B-8

• Notify the COR if bear, fox, wolverine, fish, or wolf are encountered in the project area.

Notify the COR if the bears and foxes are seeking human food.

• Store all food, toiletries, and other potential attractants (e.g., petroleum products, antifreeze, personal hygiene items) in wildlife-proof containers or enclosed construction equipment, except during actual use.

• Provide temporary portable toilets for construction staff use. Do not place within 50 feet of surface waters. Secure toilets to prevent tipping over from winds.

• Report wildlife collisions or roadkill at the project to COR within 1 hour of discovery.

• Contractor will have on hand equipment and materials to allow measures to be taken in the event a confined aquifer is breached. COR and the Contractor will define the materials and actions required to plug the aquifer, address excess water on the surface, prohibit any water or sediment from reaching surface waters, reduce erosion, and reduce environmental damage. All materials and equipment needed to stop artesian flow will be on site prior to drilling. Contractor will have the means to measure groundwater temperature to determine if geothermal conditions are present. Potential materials would include inflatable packers, cement grout, silt fencing, 100% natural and biodegradable fiber wattles, and equipment needed to place all

• Visually monitor the drilling operation. If visible turbidity or a sheen in the discharge is observed, stop all drilling activities and notify the COR immediately. Once monitoring results show turbidity is no longer visible, drilling activities may resume and continue to monitor.

• Bio-filter bags will be placed on the bridge deck around the borehole and drilling area to prevent drilling fluids from discharging into the river. All drill fluids, cuttings, waste, and materials cored through or used to repair the bridge deck will be contained and transported off site for disposal.
