The type of construction is generally 2‐lane roads or highways either accessing Federal lands or within Federal lands.

2. OBJECTIVE

The objective of this requirement is to support WFLHD operations by providing post-construction National Pollutant Discharge Elimination System (NPDES) Construction General Permit (CGP) compliance for the OR FLAP GRANT 24(2), Granite Hill Road Reconstruction, Phase II project.

3. SCOPE

To fulfill the objective of this contract, the Contractor shall provide support to ensure compliance with the CGP 1200-CA requirements for the project per the applicable permit as well as the requirements in this SOW. This includes but is not limited to, maintenance and updating of the project’s Erosion and Sediment Control Plan (ESCP) of record, maintenance of the NPDES Public Notice sign, completion of required inspections, implementation of corrective actions as needed, and maintenance of erosion and sediment control measures until the site meets the requirements of final stabilization (i.e., 70% revegetation).

4. PERFORMANCE SCHEDULE

The work listed in this order shall begin upon NTP and shall be completed November 30, 2027, or when the site has met the requirements of final stabilization according to the CGP and as approved by the Technical Point of Contact (TPOC), whichever is sooner.

5. PLACE OF PERFORMANCE

The Project Site is located 25 miles west of Baker City, OR in Grant County. Approximate Global Positioning System (GPS) coordinates for the beginning of the project are N=404440.44, E=8777114.37 with a project length of approximately 1 mile. This post-construction compliance phase of the project begins on Granite Hill Road at MP 10.9 and ends at MP 15.9, with 5 separate construction zones within the 5-mile stretch.

6. PERSONNEL REQUIREMENTS & QUALIFICATIONS

The Contractor shall provide a qualified Erosion Control Supervisor (ECS) and Visual Monitoring Inspector with no less than 5 years of erosion and sediment control experience, currently certified under one of the following, and qualified according to the CGP/ESCP:

Identify, in writing, the ECS, Visual Monitoring Inspector, and any erosion control team member(s) by name and position. Include their individual responsibilities according to the CGP and their qualifications in the ESCP.

To the greatest extent practicable, the Contractor shall make every effort to utilize reliable, long-term employees to perform all requirements specified for the purpose of this order. In the event of a required replacement, proposed personnel resume(s) shall be submitted to the TPOC no less than 14 days prior to the replacement. The TPOC will review the resume(s) to ensure they meet the minimum qualification requirements of this contract prior to any WFLHD job assignment. The Government will notify the Contractor within 3 days if the submitted resume(s) do/does not meet the minimum qualifications.

7. TASK DETAILS

A. Task 1. Maintain permanent and temporary measures to control erosion, sedimentation, and discharge of pollutants, according to the CGP, the project ESCP of record, and this

SOW.

Immediately report to the TPOC any incident of non-compliance with the CGP that may endanger health or the environment. Provide copies of any correspondence or reports required by either the CGP or ESCP.

Manage maintenance, inspection, and reporting for erosion and sediment control measures, maintain and update the ESCP of record, and prepare any documentation required by the CGP or ESCP.

B. Task 2. Perform ESCP inspections as required in the CGP and ESCP. Document inspections using forms provided in the ESCP of record and retain the records in the ESCP binder. Submit each inspection to the TPOC for approval. Allow 2 working days for TPOC approval of inspections. Co-sign each approved inspection and file in the ESCP binder.

Complete all ESCP forms as construction progresses until final stabilization.

Inspections shall occur no more frequently than monthly unless otherwise approved by the TPOC.

C. Task 3. During inspections, visually monitor all points where stormwater is discharged off site into a receiving water, constructed or natural site drainage feature, or storm drain inlet.

If runoff has visible turbidity, conduct turbidity sampling, using a calibrated turbidity meter.

Calibrate prior to sampling and on the same day of sampling.

Identify all sampling locations in the ESCP and record sampling results in the inspection forms. Conduct turbidity sampling as follows using a calibrated turbidity meter and record in a daily log the date, time, location and turbidity levels at each monitoring location:

i. BACKGROUND LOCATION. Collect a sample at a relatively undisturbed location, approximately 100 feet up current from the disturbing activity and/or stormwater discharge location.

D-3

Granite Hill Road Reconstruction, Phase II

ii. DISCHARGE LOCATION. Collect a sample at the point where stormwater runoffruns off and enters the surface water.

If turbidity as measured in Nephelometric Turbidity unit (NTU) is no more than 10 percent higher compared to the background location sample taken upstream, no further monitoring is required.

If turbidity exceeds 10 percent of background, notify TPOC immediately. Take immediate action to address cause of exceedance. Continue to monitor until the stormwater discharge is measured at less than 10 percent of background NTU. For quotation purposes, this statement of work does not cover more than 1 day of monitoring of turbidity exceedance.

If turbidity does not resolve the same day as the inspection, immediately notify the TPOC to request a determination regarding a modification to increase the scope. To the greatest extent practicable, provide a revised quote within 48 hours of TPOC notification which shall address the additional work required.

D. Task 4. Maintain the functionality of erosion and sediment control measures, and clean measures that are one-third full of sediment, until disturbed sites are stabilized according to the TPOC. Remove and dispose of accumulated sediment according to all permit requirements. Update the ESCP as needed if changes are made to erosion and sediment control measures. For quotation purposes, assume that any maintenance will be completed the same day as the inspection using hand tools only. This statement of work does not cover more extensive maintenance or use of specialized equipment.

If maintenance activities cannot be completed as stated above, immediately notify the TPOC to request a determination regarding a modification to increase the scope. To the greatest extent practicable, provide a revised quote within 48 hours of TPOC notification which shall address the additional work required.

E. Task 5. Identify erosion and sediment control measures that are damaged or failing.

Implement corrective actions and amend erosion and sediment control measures as needed based on the site inspections to ensure compliance with the conditions of the permit. Update the ESCP as needed. For quotation purposes, assume that corrective actions could will be completed the same day as the inspection using hand tools only.

This statement of work does not cover more extensive repairs or use of specialized equipment.

If damage or failure is found which isto be so extensive that it cannot be repaired as stated above, immediately notify the TPOC to request a determination regarding a modification to increase the scope. To the greatest extent practicable, provide a revised quote within 48 hours of TPOC notification which shall address the additional work required.

D.F. Task 6. After the site has reached final stabilization, take photographs to demonstrate permanent stabilization. All photos must be clear and in focus and in the original format and resolution. Include the date each photograph was taken, coordinates for each photo location, and a brief description of the area of the site captured by the photograph. Include copies of photographs in the ESCP.

D-4

Granite Hill Road Reconstruction, Phase II

E.G. Task 7. After the site has reached final stabilization, and as confirmed by the TPOC, remove and dispose of temporary erosion and sediment control measures and the Public Notice Sign.

F.H. Task 8. Provide the TPOC with the complete ESCP of record, including inspection forms, logs, photographs, and all other required documentation according to the CGP.

8. PROGRESS REPORTS

The Contractor shall submit monthly progress reports to the TPOC. These reports shall be prepared and submitted by the 10th of the month following the month the inspection was conducted.
