(a) The Government will pay the Contractor for the full performance required under the terms of this order. The ceiling price of the order is an estimate of the total work required to complete the project tasks. The Contractor will be paid for actual work performed and/or supplies received. Work and/or supplies not received will not be compensated.

(b) For firm-fixed-price (FFP) contracts where supplies are delivered or services are performed, the final invoice is not subject to further contract settlement actions between the Government and the Contractor.

(c) For non-FFP contracts, further contract settlement actions may exist where final invoices are not to be submitted yet until these contract settlement actions are resolved. For these contracts, the Contracting Officer will process final invoices as a routine part of the closeout process. When requested, the Contractor shall submit the final invoice directly to the Contracting Officer/ Contract Specialist along with the final closeout documents. When the final invoice is ready for payment, the Contracting Officer will inform the Contractor that the final invoice can be submitted through the iSupplier system.

FHWA-002 -- Post-Award Evaluation of Contractor Performance

The CPARS module assesses performance on contracts for Systems, Services, Information Technology, and Operations Support; Architect-Engineer contracts; and Construction contracts. Reference material can be accessed in CPARS.

(e) The registration process requires the Contractor to identify an individual that will serve as a primary contact. This individual will be authorized access to the evaluation for review

C-14

Snug Harbor Road Gulley Remediation and Erosion Control and comment. In addition, the Contractor is encouraged to identify a secondary contact in the event the primary contact is unavailable to process the evaluation within the required 60-day time period. After the FHWA Focal Point registers the contract in CPARS, the contractor representative will receive a system-generated email notifying him/her that the contract is registered. Snug Harbor Road Gulley Remediation and Erosion Control

SECTION D – STATEMENT OF WORK

1. BACKGROUND

Western Federal Lands Highway Division (WFLHD), a division of the US Department of Transportation – Federal Highway Administration, designs and administers, primarily, road construction projects on Federal Lands in the states of Alaska, Washington, Oregon, Idaho, Montana, and Wyoming. The type of construction is generally 2‐lane roads or highways either accessing Federal lands or within Federal lands.

A prior WFLHD project (Snug Harbor Road Paving and Bike Lane Phase II) in this area had laid back the slope to 1V:2H and underdrains were installed on the slope, draining to the roadway ditch. The slope was then covered with 4-inches of topsoil and a rolled erosion control product (type 2.D) was installed and hydroseed was applied. However, shortly after completing the work, a significant rain event occurred that caused surficial erosion and gulley formation.

2. OBJECTIVE

The objective of this procurement action is to procure remediation on gullies that have formed (without placing equipment on the slope) and clean-up some of the existing erosion control product placement to reduce the risk of further erosion of the slope.

A. Location of Work. The Project Site is in Cooper Landing, Kenai Peninsula Borough, AK along the Snug Harbor Road. This third phase of the project begins on Snug Harbor Road approximately 1.2 miles east of Sterling Hwy and ends .23 miles to the east of that.

D-2

B. General Requirements

i. Pre-Work Conference. A minimum of two weeks prior to commencement of work, Contractor is required to participate in a pre-work conference. Contractor shall be prepared to discuss their schedule, how they anticipate performing the work, submission/approval of the needed lane closure permit from Alaska DOT&PF, subcontractors used, and submittals.

ii. Submittals. Select material sources and submit information indicating the source and certifications verifying that the proposed material meets the required specifications. Do not incorporate material requiring submittal into the work until approved.

Submit the following for review and approval at least 7 days before installing:

a. Topsoil

b. Straw Fiber Rolls

c. Short-Term Double-Net Erosion Control Blankets, Type 2D

d. Fertilizer

e. Biotic soil amendment

f. Seed

g. Tackifier

h. Bonded Fiber Mulch

iii. Hazardous Spill Plan. Submit and follow a Hazardous Spill Plan when a Spill

Prevention, Control, and Countermeasure plan is not in effect. Submit the Hazardous Spill Plan at least 2 days before beginning work. Develop a plan describing what actions will be taken in case of a spill and incorporate preventative measures to be implemented (such as the placement of refueling facilities, storage, and handling of hazardous materials, etc.).

iv. Due to the instability of the project location, the existing slopes cannot be further destabilized by equipment. Therefore, heavy equipment will not be allowed on the slope to perform this work.

v. Storage of Material and Equipment. Use only approved portions of the right-of-way for storing material or equipment. Provide additional space as needed. Do not use private property for storage without written permission of the owner or lessee.

vi. Cleaning of Vehicles and Equipment. Remove dirt, plant, and foreign material from vehicles and equipment before entry into project and material source. Prevent introduction of noxious weeds and non-native plant species into the work site.

Follow applicable Federal land management agency requirements and state requirements. Maintain cleaning and inspection records. Clean hauling vehicles before their initial entry; subsequent entries will not require cleaning unless requested. Notify the CO a minimum of 48 hours before entry to allow for inspection.
