About this file
This is a Contractor Compliance Agreement form (DOC-2818) issued by the Wisconsin Department of Corrections, Division of Management Services. The form serves as a standardized compliance certification document that contractors must complete and submit to demonstrate their commitment to adhering to all contract terms and conditions when providing services to the Wisconsin Department of Corrections.
The agreement requires contractors to certify compliance with two primary areas: general contract compliance ensuring all services are delivered consistent with DOC contract requirements, and specific compliance with background check provisions for all contractor staff and applicants. The form establishes a twelve-month compliance period from the date of signature, covering all current and future DOC contracts during that timeframe. Contractors must renew this compliance certification annually by filing a new completed form within 30 days of each contract year end date or within twelve months of the previous filing, whichever occurs earlier. The document does not specify particular goods, services, or pricing terms as it serves as a general compliance framework applicable to all DOC contractor relationships.
