About this file

This is an Application and Applicant Certification Form for a Request for Qualifications (RFQ) issued by the Oregon Department of Human Services, Office of Adults and People with Disabilities for Environmental Modification Services (RFQ # S-10000-00015326). The RFQ seeks qualified construction contractors to perform environmental modifications in consumers' homes throughout Oregon to ensure health, welfare, safety, and enable greater independence. Applicants must complete the certification form by providing company information, contact details, tax identification, and Oregon Secretary of State registration status. The form requires documentation of minimum qualifications including active Construction Contractors Board (CCB) licensing, Building Codes Division plumbing and electrical licenses (if applicable), relevant certifications such as asbestos abatement or lead paint certification, and availability to serve specific Oregon counties. Applicants must declare their experience in environmental modification services and identify the types of modifications they are qualified to perform, including wheelchair ramps, ceiling track installation, stair lifts, residential elevators, grab bars, doorway widening, bathroom modifications, kitchen accessibility modifications, bedroom adaptations, exterior accessibility features, roofing, and general construction.

The certification requirements mandate that applicants possess appropriate insurance coverage as specified in the sample contract, have no unpaid debts or CCB complaints, maintain good standing with no terminated or canceled agency contracts, and comply with all federal and state tax laws and nondiscrimination requirements. Applicants must certify compliance with Oregon employment laws, wage and hour protections, and anti-retaliation policies, and confirm they are not listed on federal exclusion lists maintained by the Treasury Department's Office of Foreign Assets Control or the System for Award Management. All applicants must submit supporting documentation including the completed application form, a Certified Disadvantaged Business Outreach Plan, a Responsibility Inquiry form, copies of all licenses and applicable certifications, and sign the certification attesting that all information is truthful, accurate, and prepared independently without collusion or fraud. The application requires an authorized representative's signature confirming acceptance of RFQ requirements and the sample contract terms and conditions.
