About this file
This is a sample contract document prepared by the Oregon Department of Human Services, Office of Adults and People with Disabilities for environmental modifications services. The contract outlines the framework for qualified construction contractors to perform home modifications for consumers to ensure their health, welfare, safety, and independence. The services encompass structural alterations, accessibility improvements, and safety enhancements tailored to individual consumer needs within their residential environments. The contract establishes the terms under which selected contractors will be engaged to deliver these modifications on a project-by-project basis, with compensation structured according to the scope and complexity of each modification request.
The contract specifies payment mechanisms tied to completion of defined modification tasks and includes provisions for contractor qualifications, insurance requirements, and performance standards. The document establishes the state's authority to manage contractor selection, project approval processes, and quality assurance throughout the modification work. Contractors are required to maintain appropriate licensing, bonding, and liability insurance coverage. The contract terms address dispute resolution, termination conditions, and compliance with applicable state and federal regulations governing construction work and consumer protection.
