About this file
This is an Attachment C form from the State of Oregon's Request for Proposals S-10000-00013061 for Family Search and Engagement services, administered through the Department of Administrative Services (DAS) Procurement Services. The document serves as the Proposer Information and Certification Sheet that all bidders must complete and submit as part of their proposal response. The RFP requires proposers to provide legal name, address, state of incorporation, entity type, contact information, and Oregon Business Registry Number if applicable. Proposers must acknowledge receipt of all RFP addenda, confirm that their proposal constitutes a firm offer valid for 180 days following the closing date, and commit to performing the scope of work and meeting performance standards in the final negotiated contract. The full proposal submission deadline is September 30, 2025.
The certification requirements outlined in this attachment establish mandatory compliance standards for all proposers, including verification of tax law compliance with Oregon Revised Statutes; non-discrimination in employment and subcontracting practices; compliance with wage and hour requirements under ORS 652.220; verification that neither the proposer nor its employees, agents, or subcontractors appear on federal exclusion lists maintained by the Office of Foreign Assets Control or the System for Award Management; disclosure of any actual or potential conflicts of interest; and certification that all proposal contents are truthful, accurate, and prepared independently without collusion or fraud. Proposers with anticipated contract values of $150,000 or more must certify implementation of written policies and practices to prevent sexual harassment, sexual assault, and discrimination against protected class employees. The form also requires proposers to acknowledge that false statements or misrepresentations in response to the RFP may constitute false claims subject to the Oregon False Claims Act and associated liabilities and penalties.
