About this file

This is a Vendor Questionnaire for janitorial services required to be submitted with bids for the Medical Examiner's Office in Texas. The questionnaire requests that bidders provide detailed information about their company's experience and capacity to perform the contracted services. Vendors must disclose the length of time their company has been providing janitorial services along with a description of services offered, current employee count, employee turnover rate for the past year, the number of employees to be assigned to the location if awarded the contract, and identification of the contract administrator including their title, tenure with the company, and total years of experience in the janitorial profession.

The questionnaire serves as an evaluation tool to assess vendor qualifications and stability prior to contract award. This document must be returned with all bid submissions and does not establish pricing terms, set-asides, or funding information but rather functions as a qualification screening mechanism for prospective service providers.
