About this file

This is a change request document submitted for approval to AG Brian Swan regarding a contract modification for the State of Utah Department of Corrections. The document pertains to the provision of limited legal services to inmates incarcerated at the Uintah and Duchesne County Jails Facilities under the jurisdiction of the Utah Department of Corrections. The change request has been executed with the required signature authorization from the Attorney General's office, indicating formal approval of the proposed modification to the existing contract terms or conditions.

The modification addresses adjustments to the contract originally established through the Invitation for Bid process, which carried a total budget allocation of $70,000.00 over a five-year contract term. The change request represents a formal procedural step in the contract administration process, requiring approval from the State Attorney General's office prior to implementation. This signed authorization document confirms that the proposed changes have received the necessary legal review and executive approval to proceed with the contract modification.
