About this file

This document is Attachment B to a Request for Proposals (RFP) issued by Bernalillo County, New Mexico for Occupational Medical Health Services. The attachment outlines the scope of medical services and testing procedures that prospective offerors must provide pricing for, including blood work (Complete Blood Count, SMAC, HDL, and toxicology screening), audiograms, spirometry, respiratory protection medical evaluations and fit testing, EKGs, spine and chest x-rays, certificates of pre-existing impairment, and infectious disease screenings with various vaccine options. Additional services encompassed in the pricing request include office visits at multiple complexity levels (limited, intermediate, extended, complex, and comprehensive), fitness-for-duty evaluations, return-to-work assessments, urinalyses, drug screening and breath alcohol testing, tetanus and rabies vaccines, physician consultation hours, and Medical Review Officer services. The document also includes pricing for pesticide and rodenticide testing on an as-needed basis.

Offerors must submit proposed rates for all 19 line items and service categories listed in the attachment. The pricing structure requires individual rates for each medical service and procedure, allowing offerors to propose alternative toxicology screening methods if desired. The cumulative total of all proposed rates must be included at the bottom of the attachment. This cost proposal attachment directly supports the County's need for comprehensive occupational health services including pre-employment screening, on-the-job injury treatment, OSHA-governed physicals, and fitness-for-duty evaluations to be provided to County employees and operations.
