About this file
This document is an Amendment (Amendment 4) to a Request for Proposal (RFP No. 2089620) issued by Dallas Area Rapid Transit (DART) in Dallas, Texas for Workers' Compensation Administrative Program Services. The amendment is effective August 15, 2025, and provides detailed responses to 17 vendor questions (questions 20-36) regarding proposal submission requirements, page limits, electronic submission procedures, and clarifications about various exhibit instructions. The proposal submission deadline remains August 19, 2025, at 2:00 P.M. Central Time, with all proposals to be submitted electronically through the Bonfire portal.
The amendment includes supplemental data about DART's current workers' compensation environment, revealing historical claims and preauthorization trends. Specifically, the document shows preauthorization/utilization review requests have decreased from 618 in 2022 to a projected 418 in 2025, and claims data indicates fluctuating incident and medical claim volumes across calendar years 2021-2025. The proposal requires submission of specific forms including Form 1295, and vendors are instructed to provide technical proposals not exceeding 150 pages, excluding DART-required forms. No contract extensions will be granted, and the amendment emphasizes strict adherence to submission guidelines, with warnings that failure to acknowledge the amendment could result in offer rejection.
