About this file
This file is an Anti-Lobbying Certification Form (Exhibit B) and Disclosure of Lobbying Activities form (Standard Form LLL) required for bids or offers exceeding $100,000 submitted to the Granite School District in Utah for Solid Phenolic Bathroom Partitions at Pioneer Elementary. The form serves as a certification that no Federal appropriated funds have been or will be paid to influence officers or employees of any agency, members of Congress, or Congressional employees in connection with the Federal contract or award. Bidders must submit this certification as a prerequisite to the transaction, and failure to do so subjects the bidder to civil penalties ranging from $10,000 to $100,000 for each failure. The form requires the contractor's authorized official to certify the truthfulness and accuracy of all statements and acknowledge that the provisions of 31 U.S.C. 3801 et seq. apply to the certification and disclosure.
The Disclosure of Lobbying Activities form (Standard Form LLL) must be completed if any funds other than Federal appropriated funds have been or will be paid to any person for lobbying purposes related to the covered Federal action. The form requires detailed reporting of lobbying entities, individuals performing services, amounts and forms of compensation, types of payments, and a detailed description of services rendered. All subrecipients and subawardees at all tiers must include the same certification language in their award documents and certify and disclose accordingly. The reporting entity must identify the type of Federal action, status, report type, Federal agency, program name, and award amount, with submission required at the initiation of the covered Federal action or upon material changes to previous filings. Instructions specify that the form is authorized for reproduction and that information reported will be disclosed to Congress semiannually for public inspection.
