About this file

This is a vendor instruction document for Form 1295, required by Texas law for all vendors submitting proposals to Klein ISD. The form mandates disclosure of any interested parties who have a controlling interest in the vendor's business or who help negotiate or facilitate the contract with Klein ISD, including owners, officers, brokers, advisers, and attorneys. Every vendor must complete, print, sign, and attach the certified Form 1295 to their proposal, even if no interested parties exist. The specific contract referenced in this instruction is RFP 26-102 for Professional Development services. Vendors are required to complete the form online through the Texas Ethics Commission website, obtain a certification number, sign the completed form, and submit it with their bid. Section 3 of the form must include the contract identification number (RFP 26-102) and bid name/description (Professional Development).

Form 1295 applies to all business entities—including sole proprietorships, partnerships, and corporations—contracting with Klein ISD, with limited exceptions for sponsored research contracts at universities, interagency contracts between state agencies or universities, and certain health and human services contracts of unknown value. The form requires notarization and affidavit certification under penalty of perjury, with vendors either identifying all interested parties with controlling or intermediary interests or certifying that no interested parties exist. Vendors must add additional pages as necessary to disclose all applicable interested parties, and the completed form becomes part of the official procurement record maintained by the Klein ISD Procurement Administrator.
