About this file
The document is a Scoring Form (Exhibit 5) from the City of Dallas for evaluating contract applications, specifically for a program related to human trafficking services. The scoring form is structured across three main categories: Experience (40 points), Approach (30 points), and Program Budget, Cost, and Timeframe (30 points), totaling 100 points. The evaluation criteria assess an organization's capability to provide services, including document submission, prior experience with the target population, program development and management skills, staff qualifications, and past performance with city contracts.
The scoring methodology emphasizes comprehensive organizational assessment, with points allocated for demonstrating clear processes for client eligibility, articulating a detailed work plan, describing community engagement strategies, and presenting a budget that supports proposed services. The document notes that while leveraged resources are welcome, there is no minimum funding requirement. The evaluation process appears designed to select a project lead who can effectively serve a population affected by human trafficking, with a focus on the organization's ability to develop, manage, and measure the impact of the proposed program within a one-year contract term.
