About this file
This document is a City of Dallas Activities Report (Exhibit 2) designed to track and report on services provided to human trafficking survivors. The report is a comprehensive template for organizations to document their work in supporting survivors through wrap-around social services, economic empowerment strategies, and ongoing barrier mitigation. The form captures detailed demographic information, types of victimization, direct services provided, and outcomes for participants, with sections for tracking new and cumulative individuals served throughout the year.
The report focuses on three primary service areas: 1) Supporting survivors with wrap-around social services including emergency shelter, housing, trauma-informed counseling, and basic necessities; 2) Implementing economic empowerment strategies such as financial advocacy, workforce readiness, resume development, and job matching; and 3) Ensuring survivors maintain employment through ongoing case management, childcare support, and professional development resources. The form requires detailed tracking of participant demographics, including race, ethnicity, gender, age, and special classifications like veterans, immigrants, or individuals with disabilities. Additionally, the report captures critical economic metrics such as GED completion, vocational training, employment rates, and changes in monthly income, demonstrating a holistic approach to survivor support and economic rehabilitation.

DEFINITIONS
The five racial categories are defined as follows:
a) American Indian or Alaska Native. A person having origins in any of the original peoples of
North and South America (including Central America), and who maintains tribal affiliation or community attachment.
b) Asian. A person having origins in any of the original peoples of the Far East, Southeast Asia, or the Indian Subcontinent including, for example, Cambodia, China, Japan, Malaysia, Pakistan, the Philippines Islands, Thailand, and Vietnam.
c) Black or African American. A person having origins in any of the black racial groups of Africa.
d) Native Hawaiian or Other Pacific Islander. A person having origins in any of the original peoples of Hawaii, Guam, Samoa, or other Pacific Islands.
e) White. A person having origins in any of the original peoples of Europe, the Middle East, or
North Africa.
The two ethnic categories are defined as follows:
a) Hispanic or Latino. A person of Cuban, Mexican, Puerto Rican, South or Central America, or other Spanish culture or origin, regardless of race. The term, "Spanish origin," can be used in addition to "Hispanic or Latino."
b) Not Hispanic or Latino. A person not of Cuban, Mexican, Puerto Rican, South or Central American, or other Spanish culture or origin, regardless of race.
Victimization type:
Human Trafficking: The use of force, fraud, or coercion to compel a commercial sex or labor act, or in which the person induced into commercial sex is a minor. The Trafficking Victims Protection Act of 2000 and its subsequent reauthorizations recognize and define two primary forms of human trafficking (U.S. Department of Justice):
a) Sex trafficking is the recruitment, harboring, transportation, provision, obtaining, patronizing, or soliciting of a person for the purpose of a commercial sex act in which a commercial sex act is induced by force, fraud, or coercion, or in which the person induced to perform such act has not attained 18 years of age. (22 U.S.C. § 7102(11)(A)).
b) Forced labor is the recruitment, harboring, transportation, provision, or obtaining of a person for labor or services, through the use of force, fraud, or coercion for the purpose of subjection to involuntary servitude, peonage, debt bondage, or slavery. (22 U.S.C. § 7102(11)(B)).
c) While the above are the legally recognized definitions of human trafficking, the City recognizes not all cases of victimization rise to the level where it is prosecuted as such. Accordingly, for this RFP, eligible survivors include adults (18 and up) who are high-risk, those who are past or current victims (based on the agency’s individual assessment), and those who the agency suspects are victims but no outcry by the victim has been made.
