About this file
This is a Past Performance Reference Information Form template for the BPM Cables (Semi Rigid) for ALS-U solicitation (AA-2026-04) issued by the Department of Energy on behalf of Lawrence Berkeley National Laboratory. The form is limited to three pages per reference contract and requires offerors to provide comprehensive documentation of relevant past performance.
The form requires offerors to submit the following information for each reference contract: offeror name and UEI, reference client name and UEI, contracting entity details, contract number and title, client point of contact (including Contracting Officer or equivalent), period of performance, contract type, total contract value with average annual value, and value performed to date as of the RFP release date (April 13, 2026). Additionally, offerors must provide a summary of work complexity including scope, difficulty, similarity to the current work, and any problems encountered with creative solutions implemented. Critically, the form mandates disclosure of safety statistics and records for both the offeror and any proposed lower-tier subcontractors, including three-year Experience Modification Rate (EMR) data from workers' compensation carriers, with any EMR above 1.0 requiring explanation. Offerors must fully disclose all OSHA, Cal-OSHA, and state occupational safety and health citations received within the past three years by the offeror or proposed subcontractors, including violation category, specifics, and current status. Failure to disclose safety violations shall render the offer non-responsive and may be cause for default if discovered post-award. Proposals are due no later than April 27, 2026.
