About this file

This is a Non-Conflict of Interest certification form required by the Colorado Department of Corrections for bid submission on a Bed Sheeting Fabric contract opportunity. The form mandates that bidders certify compliance with Colorado Revised Statutes sections 18-8-301, 18-8-302, 18-8-306, and 18-8-308, as well as the State of Colorado State Controller Policy on Procurement Conflicts of Interest. Bidders must affirm they have no actual, potential, perceived, or apparent conflicts of interest and acknowledge the Procurement Official's authority to remove them from the process. The form requires signed certification from an authorized representative of either a private employer or an independent contractor under contract with the State of Colorado.

The certification document establishes that bidders understand they may have access to State information and records during the procurement process, which may only be used as expressly authorized and cannot be distributed, sold, or shared with third parties. Signatories must commit to immediately notifying the Procurement Agent if any situation arises that could alter their representations or create an appearance of conflict or impropriety. This form must be completed and returned with the bid submission and serves as a prerequisite for participation in the Colorado Department of Corrections procurement process.
