About this file
This is a Non-Conflict of Interest certification form required by the Colorado Department of Corrections for bid submissions. The document serves as a declaration by bidders or their authorized representatives confirming compliance with Colorado Revised Statutes sections 18-8-302, 18-8-306, 18-8-301, and 18-8-308, as well as the State of Colorado State Controller Procurement Conflicts of Interest Policy. Bidders must attest that they have no actual, potential, perceived, or apparent conflicts of interest related to the procurement opportunity and understand that the CDOC Procurement Official retains the authority to remove them from consideration at any time or grant removal requests as deemed appropriate.
The certification requires bidders to acknowledge their understanding of information governance restrictions, confirming that any State information or records accessed during the procurement process will not be distributed, sold, or shared with third parties except as expressly authorized. Bidders must also commit to immediately notifying the Procurement Agent of any situations that could alter their representations or create the appearance of conflict or impropriety. The form requires completion and return with the bid submission, including printed name, signature of an authorized signer, and the company name or doing business as designation. This form is mandatory documentation for participation in the CDOC procurement process.
