About this file

This document is a Past Performance Reference Information Form for Request for Proposal (RFP) JG-2025-01, designed to capture detailed historical performance information from potential contractors. The form requires comprehensive documentation of previous contract experiences, including contract client details, contact information, contract value, performance period, and contract type. A critical section requires full disclosure of safety statistics, specifically mandating reporting of Experience Modification Rate (EMR) for the past three years and complete documentation of any Occupational Safety and Health Administration (OSHA) or California Occupational Safety and Health Administration (Cal-OSHA) citations. The form emphasizes transparency, warning that failure to disclose safety violations could render an offer non-responsive or cause for default, and allows offerors to modify the form's format while maintaining specified informational, font, and page limitations.
