About this file
This is a cost proposal form for solicitation BB26-20 issued by the State of Utah Division of Purchasing in conjunction with the Department of Health and Human Services. The RFP seeks to procure two GSI Corti DPOAE hearing screeners through a single purchase order contract award. The contract term is one year, with the vendor required to guarantee pricing for the entire period. The solicitation will result in an award to the responsive and responsible vendor with the highest overall score, as justified by the procurement code.
The cost proposal must include all implementation and annual maintenance costs necessary to fulfill the scope of work, including two GSI Corti DPOAE hearing screeners, one DPOAE probe replacement, and annual calibration for one screening unit. All line items must be fully loaded with costs, including travel and any expenses required to implement and maintain the proposed solution. Pricing will be evaluated using a competitive formula wherein the offeror submitting the lowest proposed price receives 100 percent of the available cost points, and all other offerors receive a proportional percentage based on how their price compares to the lowest bid. Any costs not documented on the cost schedule will not be allowed into the resulting purchase order, and any deviation from the specified format may result in disqualification of the offeror's proposal.
