About this file

This is a Mandatory Minimums and Technical Criteria evaluation document for Request for Proposal BB26-20 GSI Corti 2, issued by the State of Utah Division of Purchasing in conjunction with the Department of Health and Human Services. The procurement seeks to obtain two GSI Corti DPOAE hearing screeners from a responsive and responsible vendor. The equipment must be deliverable by the deadline of Friday, March 20th, 2026. The resulting award will be a single purchase order with a contract term of one year, during which the vendor must guarantee its pricing.

Evaluation of proposals will be based on four criteria: Equipment Features (250 points), Ability to Meet Scope of Work and Delivery Deadline (250 points), Customer Service (200 points), and Cost (300 points, representing 30 percent of the total evaluation). Mandatory minimums require that offerors must be able to provide services in the State of Utah, must submit a cost for the two GSI Corti DPOAE hearing screeners or equivalent equipment, and must respond to three technical criteria proposal questions in the RFP's Questions Section. The Equipment Features evaluation will assess OAE probe tip type, signal-to-noise ratio for pass results, included accessories, and available add-on accessories. Customer Service evaluation will focus on the vendor's described service practices and protocols.
