About this file
This is an Interior Path-of-Construction (POC) Asbestos Survey Report prepared by Terracon Consultants, Inc. for the Smith Clinic located at 2525 A Holly Hall Street in Houston, Texas. The survey was conducted on September 16, 2025, by Texas Department of State Health Services (TDSHS) licensed asbestos inspectors Christian H. Pedraza and Jonathan Hernandez in accordance with Environmental Protection Agency (EPA) regulations 40 CFR 763 (AHERA), Texas Asbestos Health Protection Rules (TAHPR), and OSHA standards. The survey identified and documented asbestos-containing materials (ACM) within the interior path-of-construction of three renovation projects: SCP ACS SC Linear Accelerators Replacement (Rooms 1302/1302A, 1303/1303A, and 1304/1304A), SCP ACS SC CT Scans (1-3) and CT Simulator Replacement (Rooms 1605, 1607, 1609, and 1112), and SCP ASS SC RAD Fluoroscopy Replacement (Rooms 1226, 1227, and 1228). The total path-of-construction area surveyed encompassed approximately 3,550 square feet. Fifty-six (56) bulk samples were collected from sixteen (16) homogeneous areas of suspect ACM throughout the surveyed spaces and submitted to Micro Analytical Services, Inc. (MAS), a TDSHS-licensed and NVLAP-accredited laboratory, for analysis by polarized light microscopy (PLM).
The laboratory analysis, completed on September 17, 2025, determined that asbestos was not identified in any of the samples collected. The survey encompassed interior building components including painted gypsum wallboard systems, white ceramic wall tile systems, white 2' x 2' ceiling tiles, painted gypsum wallboard ceilings, wood pattern strip vinyl flooring, vinyl floor tile systems, and ceramic floor tile systems. All sampled materials, including black cove base with tan mastic, brown wood pattern strip vinyl flooring, tan vinyl floor tile systems, white ceiling tiles, and painted gypsum wallboard systems, were determined to contain no detectable asbestos. The report notes that while reasonable effort was made to survey accessible suspect materials, additional suspect ACM could potentially exist in walls, voids, or concealed areas above ceilings that were not fully accessible. The survey complies with regulatory requirements mandating asbestos identification and classification prior to renovation activities, and the findings support proceeding with the planned renovation projects at Smith Clinic without asbestos remediation concerns for the surveyed areas.

INTRODUCTION
Terracon Consultants, Inc. (Terracon) conducted an interior path-of-construction (POC) asbestos survey of selected areas of the various areas in the Smith Clinic – 1st Level located at 2525 A Holly Hall Street in Houston, Texas. The survey was conducted on September 16, 2025, by a State of Texas licensed asbestos inspector in general accordance with Terracon Proposal No. P92257965 dated September 17, 2025.
Building components in the POC were surveyed, and homogeneous areas of suspect asbestos-containing materials (ACM) were visually identified and documented. Although reasonable effort was made to survey accessible suspect materials, additional suspect but un-sampled materials could be located in walls, voids or in other concealed areas. Suspect ACM samples were collected in general accordance with the sampling protocols outlined in Environmental Protection Agency (EPA) regulation 40 CFR 763 (Asbestos Hazard Emergency Response Act, AHERA) and the Texas Asbestos Health Protection Rules (TAHPR). Samples were delivered to a State of Texas licensed laboratory for analysis by polarized light microscopy (PLM).
1.1 Project Objective
We understand the purpose of this survey was to identify and quantify ACM present within the interior POC of the SCP ACS SC LINEAR ACCELERATORS REPLACEMENT (LINEAR 1-2), SCP ACS SC CT SCANS (1-3) AND CT SIMULATOR REPLACEMENT, and the SCP ASS SC RAD FLUOROSCOPY REPLACEMENT (RF ROOMS 1,2), prior to renovation. POC areas were depicted on drawings provided to Terracon by the client. TAHPR, Senate Bill 509 and EPA regulation 40 CFR 61, Subpart M, National Emission Standards for Hazardous Air Pollutants (NESHAP) require that an asbestos survey be performed prior to issuance of a city construction permit and prohibit the release of asbestos fibers to the atmosphere during renovation or demolition activities. The asbestos NESHAP requires that potentially regulated ACM be identified, classified and quantified prior to planned disturbances or demolition activities. The Occupational Safety and Health Administration (OSHA) has promulgated a worker protection standard for the disturbance of materials containing asbestos during demolition and renovation projects.
1.2 Limitations
Terracon assessed multiple areas above the drywall and ceiling systems, where possible, but did not observe additional suspect materials. Access to areas above the ceiling was limited, and the potential exists for additional suspect ACM to be present in these inaccessible areas.
Terracon lifted floor coverings in several areas in the building but did not observe additional floor coverings/layers; however, as Terracon could not assess beneath all floor coverings in
September 19, 2025 | Terracon Project No. 92257965
Facilities | Environmental | Geotechnical | Materials 2 all areas, there may be isolated areas of additional suspect material present beneath existing coverings.
Interior building components were included in the survey. Destructive sampling was conducted as indicated in Terracon’s proposal.
Terracon did not have any inaccessible areas and was able to collect samples throughout the POC areas

FIELD ACTIVITIES
Mr. Christian H. Pedraza, a Texas Department of State Health Services (TDSHS) licensed asbestos inspector (License No. 60-4222), and Mr. Jonathan Hernandez, a TDSHS licensed asbestos inspector (License No. 60-4222), conducted the asbestos survey in general accordance with the sample collection protocols established in AHERA. Copies of their licenses are is attached in Appendix E. The following sections present a summary of survey activities.

Facilities | Environmental | Geotechnical | Materials 3
3.1 Visual Assessment
Our survey activities began with visual observation of the POC to identify homogeneous areas of suspect ACM. A homogeneous area (HA) consists of building materials that appear similar throughout in terms of color, texture and date of application; building materials identified as glass, wood, fiberglass, metal or rubber were not considered suspect ACM. The assessment was conducted throughout visually accessible areas of the POC.
3.2 Physical Assessment
Terracon conducted a physical assessment of each HA of suspect ACM to assess the friability and condition of the materials. The EPA defines a friable material as a material, when dry, can be crumbled, pulverized or reduced to powder by hand pressure. Terracon assessed friability by physically touching suspect ACM.
3.3 Sample Collection
Based on results of the visual observation, Terracon collected random bulk samples from each HA of suspect ACM observed in general accordance with AHERA sampling protocols. Sample team members collected bulk samples using wet methods as applicable to reduce the potential for fiber release. Samples were placed in sealable containers and labeled with unique sample numbers using an indelible marker.
Fifty-six (56) bulk samples were collected from sixteen (16) homogeneous areas of suspect ACM. A summary of suspect ACM samples collected during the survey is included in the chain-of-custody (COC) in Appendix B.
3.4 Sample Analysis
Bulk samples were submitted under COC to Micro Analytical Services, Inc (MAS) of Houston, Texas, for analysis by PLM per EPA methodology EPA/600/R-93/116. The percentage of asbestos, where applicable, was determined by microscopic visual estimation. MAS is accredited under the National Voluntary Laboratory Accreditation Program (NVLAP Accreditation No. 200618-0) and licensed by the TDSHS (TDSHS No. 30-0341).
REGULATORY OVERVIEW
Title 25, Part 1, Chapter 296, TAHPR, regulates asbestos fiber emission and asbestos waste disposal practices for public buildings. TAHPR also require the identification and classification of existing asbestos-containing building materials prior to demolition or renovation activity.
Under TAHPR, asbestos-containing building materials are classified as either friable or
September 19, 2025 | Terracon Project No. 92257965
Facilities | Environmental | Geotechnical | Materials 4 nonfriable ACM containing 1% or more asbestos. Friable materials are those that, when dry, may be crumbled, pulverized or reduced to powder by hand pressure.
TAHPR requires that any asbestos-related activity be performed by TDSHS licensed individuals. An asbestos-related activity consists of the disturbance (whether intentional or unintentional), removal, encapsulation, or enclosure of asbestos, including preparations or final clearance activities, the performance of asbestos surveys, the development of management plans and response actions, asbestos project design, the collection or analysis of asbestos samples, monitoring for airborne asbestos, or any other activity required to be licensed under TAHPR.
TAHPR requires that abatement in public buildings be performed by a TDSHS licensed asbestos abatement contractor in accordance with a project design prepared by a TDSHS licensed asbestos consultant. In addition, a TDSHS licensed asbestos consultant agency must perform 3rd party air monitoring during the abatement activities. TAHPR requires that written notification be submitted before beginning renovation projects that include the disturbance of any quantity of ACM in a public or commercial building or facility, and before the demolition of a building or facility, even when no asbestos is present. This written notification must be provided to the TDSHS at least 10 working days prior to the commencement of asbestos abatement or demolition activities.
40 CFR Part 61 Subpart M, the asbestos NESHAP, regulates asbestos fiber emission and asbestos waste disposal practices for commercial buildings and facilities. NESHAP requires the identification and classification of existing asbestos containing building materials prior to demolition or renovation activity. Under NESHAP, building materials containing >1% asbestos are classified as either friable, Category I nonfriable or Category II nonfriable ACM. Friable materials are those that, when dry, may be crumbled, pulverized or reduced to powder by hand pressure. Category I nonfriable ACM includes packing, gaskets, resilient floor coverings and asphalt roofing products. Category II nonfriable ACM includes any nonfriable materials other than those classified as Category I materials.
Regulated asbestos-containing materials (RACM) includes all friable ACM; Category I non-friable ACM that has become friable or will be or has been subjected to sanding, grinding, cutting, or abrading; and Category II non-friable ACM that has a high probability of becoming or has become crumbled, pulverized, or reduced to powder in the course of renovation or demolition activity.
29 CFR 1926.1101, OSHA Asbestos standard for the construction industry, regulates workplace exposure to asbestos. The OSHA standard classifies construction and maintenance activities that could disturb ACM and specifies work practices and precautions employers must follow when engaging in each class of regulated work. The OSHA standard also requires that employee exposure to airborne asbestos fibers be maintained below the Permissible Exposure

Limit (PEL) of 0.1 asbestos fibers per cubic centimeter (f/cc) of air as an 8-hour Time Weighted Average (TWA).
FINDINGS AND RECOMMENDATIONS
Asbestos was not identified in the samples collected on September 16, 2025. The laboratory analytical report is included as Appendix B.
If suspect asbestos-containing materials are discovered during the renovation work that were not sampled as part of this survey, they should not be disturbed until the presence of asbestos is either confirmed or ruled out by laboratory analysis.
