About this file

This is a Proposer's General Questionnaire template from the University of Texas Health Science Center at San Antonio (UTHSCSA) Department of Purchasing for a Request for Proposal (RFP) related to Spinal Implants and related supplies for use in surgical procedures at the newly opened Multispecialty and Research Hospital (MSRH). The RFP seeks qualified vendors capable of supplying high-quality, cost-effective spinal implants, instruments, and associated products for the 144-bed facility, which includes 12 operating rooms. The scope encompasses a comprehensive range of spinal implant products including cervical, thoracic, lumbar, and sacral/pelvic spine constructs; artificial discs; rods; pedicle screws; locking screws; rod contouring systems; cross links; cages and spacers; drill bits; laminoplasty constructs; staples; and adjustable rods. The RFP explicitly excludes Kyphoplasty or Vertebroplasty Systems. This RFP will result in multi-awarded contracts to several vendors meeting the specified requirements, with no guarantee of volume, minimum, or maximum compensation provided.

The questionnaire requires proposers to provide comprehensive information including company profile details, financial statements upon request, litigation history, customer references from the past five years, key personnel resumes, service approach and philosophy, quality assurance programs, contingency and disaster recovery plans, and details regarding any unique or additional services offered. Proposers must also disclose ownership interests of 25 percent or greater and identify any relationships with UTHSCSA employees. Additionally, proposers must select one of three options regarding cloud computing services, indicating whether they will or will not provide such services in connection with the agreement.
