About this file

This is a Cost Proposal Form for Solicitation #UDOT260313MH issued by the Utah Department of Transportation (UDOT) for the Zero Fatalities PR Media Management contract. Offerors must submit cost proposals as separate documents from technical responses and must adhere strictly to the prescribed format to avoid disqualification. The proposal requires submission of a blended hourly rate that serves as the maximum billable rate for all services, encompassing all labor costs for staff and profit, including account supervision, planning, consultation, and other labor expenses. Additionally, offerors must provide three position-specific labor rates: Senior Manager/Executive, Mid-Level Staff, and Support Staff. The blended hourly rate forms the basis for assigning cost points during RFP evaluation.

Cost points are calculated using a formula whereby the offeror with the lowest proposed price receives 100 percent of available price points, and all other offerors receive a proportional portion based on their price relative to the lowest proposal using the formula: Cost Points × (2 − (Proposed Price/Lowest Proposed Price)). Offerors whose proposed price exceeds double the lowest proposed price receive no cost points. Travel costs are reimbursed according to the current Utah State Travel Policy per diem rates. The resulting contract will authorize payment for time spent on work based on agreed-upon labor rates and will permit reimbursement of pre-authorized pass-through direct expenses accompanied by invoices. Offerors are explicitly prohibited from modifying or altering the cost form format, as the State relies on the standard format for accurate and fair cost comparison.

Cost Proposal Solicitation #UDOT260313MH

To be submitted as a separate document from the Technical Response document

Offeror Name:

Cost is to be submitted based on the following:

(Any deviation from this format may result in disqualification of proposal.)

Provide a blended hourly rate for the tasks described in the RFP. The “blended” hourly rate is the maximum billable rate for all services and must include all labor costs for staff, and profit. Labor may include account supervision, planning, consultation, and other labor expenses required to accomplish the needed tasks.

Blended Hourly Rate:
$________ per/hr.

Note: The Blended Hourly Rate will be used as the basis for assigning Cost Points in the RFP evaluation. The Position specific labor rates are for contracting purposes.

Additionally, provide the following specific position labor rates:

Position
Associated Rate
1. Senior Manager/ Executive
$
2. Mid-Level Staff
$
3. Support Staff
$

Purchasing will use the following cost formula: The points assigned to each offeror’s cost proposal will be based on the lowest proposal price. The offeror with the lowest Proposed Price will receive 100% of the price points. All other offerors will receive a portion of the total cost points based on what percentage higher their Proposed Price is than the Lowest Proposed Price. An offeror whose Proposed Price is more than double (200%) the Lowest Proposed Price will receive no points. The formula to compute the points is: Cost Points x (2- (Proposed Price/Lowest Proposed Price)).

IMPORTANT: Offerors may not change/modify the format of this cost form. The State relies on this cost sheet to accurately and fairly compare offeror costs. Cost Proposals received in a modified or alternate format may be disqualified.

*Costs of travel will be reimbursed at the then-current per diem rate set by the State of Utah Travel Policy, as found at: http://www.rules.utah.gov/publicat/code/r025/r025-007.htm

The resultant contracts will specifically authorize payment for time spent on work based on the agreed upon labor rates, it also is the intent of the contract to allow for the payment of pass-through direct expenses (accompanied with invoices) as pre-authorized by the project manager.
