MICU-CCU-SICU UPGRADE
Project No: 580-23-110 Scope of Work 1 Foreword
1.1 Contractor shall provide all labor, materials, tools, equipment, expertise, administration, supervision and transportation as necessary to perform all work in accordance with provided construction specifications, drawings, all documents attached to this contract, all terms and conditions of this contract, and this statement of work.
1.2 This document does not relieve the contractor of their professional responsibility to comply with all local and federal construction and environmental codes and to perform and complete a fully coordinated job.
2 General Project Description
2.1 Phase 1: Upgrade the HVAC system and the patient headwalls in MICU-CCU bedrooms on the 3rd floor.
2.2 Phase 2: Upgrade the HVAC system, sliding glass doors and the patient headwalls in SICU bedrooms on the 5th floor.
2.3 All construction work must adhere to the latest JCAHO, NFPA, and life safety requirements. All construction work must be performed without affecting medical center’s operations. Some work will be after normal hours and weekends.
2.4 Work includes construction labor and supervision for architectural, mechanical, electrical, plumbing, medical gas, radiation protection, fire protection and demolition work as required by provided construction documents. This Scope of Work document is not considered to be a comprehensive description of work details for this project. All work shall be completed per the plans/drawings and specifications attached.
3 General Construction Requirements
3.1 Prior to any mobilization and construction, the contractor shall have an approved Infection Control Construction Permit, Construction Safety Plan, and an Interim Life Safety Measures Certification from MEDVAMC. Also project/construction schedule, schedule of values/prices and construction phases must be provided and approved by VA’s COR/Project Engineer and/or Contracting Officer.
3.2 Prior to arriving at the facility all Contractor key site personnel shall have valid ID issued by federal credentialing or USAccess.
3.3 The entire project’s associated construction and temporary works shall conform to the latest VA specifications, VA Standards, energy conservation methods, EPA, OSHA, NFPA, NEC, ASHRAE, LIFE SAFETY CODE, UBC, local, Federal and industry requirements and regulations.
3.4 Project Record Drawings: The Contractor shall maintain a set of Record Drawings (field as-built drawings) to record actual construction changes during the construction process. The project record drawings shall be available for review by the VA Project Manager at all times.
3.5 The following milestone dates shall be incorporated into the contractor’s schedule.
3.5.1 Submittal Schedule and log shall be provided within ten (10) calendar days after Notice to Proceed. The contractor shall provide it to the Contracting Officer’s Representative (COR) for approval and shall include the dates for submission of Shop Drawings, Product Data, and Samples.
3.5.2 All Shop Drawings, Product Data and Samples shall be submitted and approved by the VA within sixty (60) calendar days after Notice to Proceed. The VA will provide the Contractor with written comments within 15 business days after receiving each submittal.
3.5.3 The Contractor shall submit a construction schedule within ten (10) calendar days after Notice to Proceed. The construction schedule shall be prepared in terms of calendar days (not working days). Once the construction schedule is submitted by the Contractor and approved by the CO, no lateness in the schedule shall be tolerated without the CO’s approval.
3.5.4 A pre-construction meeting shall be held within ten (10) calendar days after award of Contract. During the construction, the Contractor shall attend all weekly or bi-weekly construction progress meetings. The meeting schedule and place will be discussed in the pre-construction meeting.
4 General Contract Requirement
4.1 VA’s Normal Working Hours are 7:00 am to 4:30 p.m., Monday through Friday. Contractor shall coordinate work with the COR in order to not disrupt the end user’s operations which may include work earlier or later in the day and on weekends or federal holidays at no additional cost to the VA. Noise disturbances and utility shutdowns/tie-ins shall be coordinated with the COR and not interfere with VA daily operations.
4.2 Pursuant to clause 52.236-8 Other Contracts incorporated by reference under section 4.33 52.252-2 Clauses Incorporated by Reference, Contractor shall fully cooperate with the other contractors and with Government employees and shall carefully adapt scheduling and performing the work under this contract to accommodate the additional work, heeding any direction that may be provided by the Contracting Officer. If required by the Contracting Officer, Contractor shall, at no additional cost to the VA, coordinate with other VA’s activities including but not limited to construction activities near the job site to ensure the construction work under this project will be in harmony with those activities. This coordination shall include but not limited to the careful scheduling of work commencement, equipment mobilization etc.
4.3 The following legal holidays are observed by this Government Agency:

5 Construction Scope The following is a general overview of the contractor scope of work Before starting the demolition work, Contractor shall perform a complete and thorough survey of the MICU-CCU, SICU area and the Interstitial space above. Survey shall include electrical conduits, HVAC ducts, plumbing for domestic water, plumbing for HVAC, and telecommunication cables. Contractor shall perform a TAB in the areas served by AHU 56 and 55 prior to starting any demolition. The rooms and hallways that are served by the AHU 55 must be returned to the same condition after construction. Prior to Phase 2, Contractor shall perform post construction TAB to verify that there are no changes to the areas served by AHU 55 and 56. Contractor shall perform TAB in the areas served by AHU 86 and 85 prior to starting any demolition. The rooms and hallways that are served by AHU 85 must be returned to the same condition. Contractor shall perform post construction TAB to verify that there are no changes to the areas served by AHU 85 and 86.
General Requirements and Work Plans
· Provide ICRA planning and plan review with facility Infection Control staff
· Staging areas must be planned and managed such that patient flow is not affected. Especially staging in the 3rd floor and 5th floor corridor must be avoided, this can affect patient, visitors and staff.
· Permission to work at the site during any periods other than standard work hours must be approved by the COR in advance.
· Compliance with Michael E. DeBakey VA Medical Center’s utility interruption policies.
· Compliance with Michael E. DeBakey VA Medical Center’s orientation and clearance programs.
· Provide daily broom cleaning of the job site, debris removal and appropriate disposal.
· Final extended installation cleaning. Site Sterilization.
· Replace HEPA filters as per filter manufacturer recommendation.
· Administer Safety and infection control program. Infection Control: The installation of drywall and plastic shrouds as temporary partitions to secure areas, control dust, protect adjacent areas and equipment. Include HEPA filtered exhaust negative air units and HEPA air scrubbers for infection control during the construction work. Walk off mats and shoe covers to reduce tracking.
Existing Conditions Phase 1 will be completed in three stages. Stage 1 will be demolition and reconstruction of the CCU. Stage 2 will be demolition and reconstruction of the MICU. Stage 3 will be the final closure of the MICU and CCU so that final tie-ins can be made for the mechanical, plumbing, electrical and telecom.
Phase 2 will be completed in four stages. Stage 1 will be demolition and reconstruction of the eight (8) rooms on the East side. Stage 2 will be demolition and reconstruction of the eight (8) rooms on the West side. Stage 3 will be demolition and reconstruction of the four (4) rooms in the inside. Stage 4 will be final closure of the SICU so that final tie-ins can be made for the mechanical, plumbing, electrical and telecom.
Prior to erecting infection control barriers and moving into the workspace, Contractor shall coordinate closely with the COR and the clinical staff to make sure that patient care is not affected.
In the 3rd floor MICU/CCU area there are a total of 18 bedrooms, with 9 MICU bedrooms on the East side and 9 CCU bedrooms on the West side, each including 1 isolation room with an ante room and 1 patient room on the interior of the area, the entire CCU wing (9 bedrooms) will be closed for construction during stage 1. Similarly, the entire MICU (9 bedrooms) will be closed for construction during stage 2.
In the 5th floor SICU area there are a total of 20 bedrooms, with 8 patient rooms on the East side and 8 patient rooms on the West side, each including 1 isolation room with an ante room. There are 4 patient rooms on the South side of the SICU.
The existing flooring including VCT floor and rubber cove base in all the 38 patient bedrooms rooms (18 bedroom in Phase 1 and 20 patient bedrooms in Phase 2) and corridor shall be removed entirely.
· Environmental analysis was performed by the AE, a report has been provided. Contractor shall be responsible for any additional analysis if needed.
· Existing ceiling tiles, radiant panels, HVAC ceiling diffusers, HVAC ductwork shall be removed.
· Existing patient headwall columns shall be removed from the bedrooms. Contractor shall retain and reuse any items as needed. The new column wall must have exactly the same number of ports, outlets and fittings.
· Unused electrical raceway, conduit and disconnects shall be removed. DEMOLISH ALL ELECTRICAL DEVICES IN AREA OF WORK. REMOVE CONDUIT AND CONDUCTORS BACK TO NEAREST JUNCTION BOX. LABEL AND CAP FOR RE-USE IN NEW WORK. Existing light fixtures in the area of renovation shall be removed or preserved as indicated in the electrical drawings. Contractor shall meter existing circuit panels and advise to facility if the new loads exceed capacity.
· Contractor shall trace the conductors back to the source in the circuit breaker panel, reuse the circuit breaker and relabel the breaker panel. It is very important for the facility to minimize adding new circuit breakers if the existing breakers have the rating.
· The existing fire suppression system shall be removed within the working space.
· Remove existing HVAC ducts, plumbing in the interstitial space. Support the telecom cables in the demolition area to avoid damage, install new cable supports as per specifications.
Concrete & Masonry As shown in the drawings. Contractors shall install structural supports for the headwall boom, pads to support the temporary air handlers, and structural supports for the air handler ducts and cables.
As per the drawings and OEM instructions, Contractor shall modify the structural members to attach the headwall boom.
Metals, Wood & Plastics, Thermal & Moisture Protection, Doors, Frames, Hardware and Windows, Finishes Contractor shall submit product information for the new sliding doors for the patient rooms and the motion sensors to the VA for approval before placing the order..
Specialties The column headwall will be replaced with an articulated boom headwall. Contractor shall provide the product details and drawings for approval before purchasing the product. Contractor shall account for lead times when planning product selection. The boom shall be designed such that the arms can be swung 360deg. The column shall accommodate everything that is currently on the column headwall.
Contractor shall provide spot coolers to the patient areas if there is any service disruptions.
There are patient lift motors and rails in the MICU, CCU and SICU patient bedrooms. Contractor shall remove, store properly, reinstall, and recertify the patient lift system including the rails. Reinstallation and recertification is required to be performed by an authorized dealer of the lift manufacturer.
Equipment Contractor shall provide Forklift and crane services to install the air handlers. The temporary air handler shall be electric powered, diesel powered units are not allowed to reduce pollution near the building. Contractor shall be responsible for providing structural pads for supporting the temporary air handler and supports for the duct. Contractor shall identify the nearest power outlets in the building suitable for their use and provide suitable electrical cables for the temporary air handler. Exterior wall penetrations must be sealed as per specifications.
Contractor shall submit to the COR for approval for following items:
· Removal procedures for old AHU 56 and 86.
· Installation procedures for new AHU 56A and 86A.
· Installation procedures for the temporary AHU.
Furnishings Contractor shall supply as per the architectural drawings.
Special Construction As per the architectural drawings.
Fire Suppression Fire alarm and sprinkler systems will be relocated and demolished as necessary within the space to facilitate any modification to patient room layout. Existing level of fire protection shall be maintained throughout any modified spaces. Any outage shall be coordinated with the COR and the facility. New sprinkler system work shall conform to NFPA 25 standards. The system will be commissioned subject to the requirement of Section 0191 00 General Commissioning Requirements.
Contractor shall provide temporary fire suppression equipment during construction.
Plumbing The Plumbing scope of this project is to replace the med gas columns in the patient rooms and replace with a new headwall boom. The headwall column along with the piping inside the column needs to be demolished up to the interstitial space to be able to install the current piping back to the new headwall booms in the patient bedroom.
In the patient bedrooms, install fittings and drains for the dialysis machines adjacent or below the handwash sinks, Contractor shall submit drawings for this fitting to the COR for approval.
Heat, Ventilating, and Air Conditioning AHU 56 and AHU 86 shall be removed and replaced with new. Contractor shall provide product details for approval before purchase. Contractor shall submit a rigging and handling plan to remove the old AHU 56 and installation of new AHU 56A and new AHU86A.
During construction, when portions of the areas served by the AHU and radiant panel water distribution is taken offline, the remaining area may be affected by high or low supply of air and water. Contractor shall adjust the air flow through the TUs and flow of water to the radiant panels in the areas so that the areas that are open to patients have normal temperature distribution. Contractor shall perform periodic visits to the areas that are operational and adjust the settings as required. Contractor shall be responsible to coordinate this tightly with the Contractor’s mechanical subcontractor, COR, VA Energy Control techs and the VA mechanical techs.
Contractor shall submit a rigging and handling plan for the RO water tank.
After completion of HVAC work, a test and balance of the HVAC system affected by the work shall be performed by a qualified, independent testing agency certified for such work.
Tie-in for new equipment to the Energy Management system. Contractor shall work with the energy controls technician and VA’s BAS system supplier to make sure that the new HVAC system (AHU, VAVs, sensors and all actuators) are correctly tied into the BAS system.
Contractor shall connect the air handling equipment, TU units, piping equipment and sensors to the building BAS system. If the existing thermostats and sensors are not compatible with the new system, then Contractor shall install new thermostats and sensors.
Contractor shall perform full commissioning, TAB in the areas. Contractor shall train VA technicians at the completion of each phase.
Electrical Install new panelboards and disconnects as needed for the new AHU.
Provide and install light fixtures in the equipment room as shown in design drawings.
Contractor shall carefully retrace the conductors back to the source circuit breaker. Perform metering in the panel boards to verify additional loads are within the rated capacity. Terminate at the nearest junction box, label and reuse. Re-Label the panel boards accordingly.
Telecommunications, Electronic Safety and Security Provide and install key card controlled electronic door lock to match existing facility devices.
Existing electronic safety and security devices shall be removed and reinstalled as necessary for the production of the work. It is Contractor’s responsibility to work with the VA’s contractors and ensure seamless interface of electronic locks, cameras and PIV readers.
Contractor may need to relocate and reroute the telecommunication cables in the interstitial spaces to make room for the new HVAC duct and new piping. Contractor shall trace the telecommunication cables from origin to destination and make sure there will not be any interruption to the service outside the construction zone. The cable supports must comply with the specifications.
6 Project Meetings
6.1 Each week the contractor shall hold a progress meeting to review progress to date and to resolve all questions for the upcoming week. The progress meeting shall include at a minimum, the Project Manager and the Superintendent representing the Contractor, the COR and the CO representing the Government. Technical supervisor (foreman), sub-contractors, suppliers, etc. shall attend when requested by the Contracting Officer. The meeting shall be same day, time, and place each week.
6.2 During construction, each day in the morning the crew and site supervisor shall hold tool box talk, the activities for the day, ICRA and safety shall be discussed during the toolbox talk.
6.3 Meeting Minutes - The contractor shall take minutes of all meetings held relative to this project. The format must be acceptable to the Contracting Officer (CO). Minutes must include the meeting date and time; agenda; meeting location; list of participants with their affiliations, and telephone numbers; be fully descriptive of issues, problems and decisions made; and action items with names of responsible parties and deadlines. In addition, the minutes must include, as attachments, all exhibits and references distributed as handout materials, unless the materials are part of a formal submission. The contractor shall provide, via e-mail, complete copies of all minutes to all attendees within 72 hours of the meeting. All challenges to the minutes will be reflected in a revised version prepared by the contractor and CO along with the contractor and COR shall sign/initial the minutes. In the event of any disputes or challenges that can’t be resolved, the Contracting Officer shall render a final decision. Copies of updated minutes shall be distributed to the all attendees within 48 hours after change notice.
7 Safety and Security
7.1 Contract workers under this project may require a VA issued identification card before the start of construction work at the Houston VAMC. There is no charge for the badges; however, multiple trips to MEDVAMC facility maybe required.
7.2 Contract employees, including all subcontractor employees, must be TB tested and receive a flu shot (each year under the contract performance) if they will work within the VA Hospital Building 100. A letter from the prime contractor shall be submitted to the CO prior to work commencing that all staff members are TB negative and the flu shot was received.
7.3 All contract personnel are subject to inspection of personal effects when entering or leaving the project site.
7.4 Parking is rigidly controlled throughout the Medical Center. Contractor parking is only allowed in authorized areas. Contractor shall abide by all police requirements. Parking in designated patient parking areas is strictly prohibited. Parking for unloading equipment necessary to perform the work will be authorized in advance by the COR prior to starting the project.
