1. Question: What is the make/model of the existing patient lifts that are required to be removed, stored, re-installed and re-certified?
Answer:

1. Question: Will the VA provide an on-site location to store the patient lifts?
Answer:
Yes, VA will provide on-site location within the construction zone, and in the interstitial space above where the hanger beams and lifts can be stored safely.
1. Question: Will the structural support shown per note 10 on sheet SF101/ SF103 be deducted as part of alternate #3?
Answer: No SF101 and SF301 are base scope and not part of the deduct.
1. Question: Keynote 53 on sheet MH104 calls for new exhaust fan support pads. Detail 13/M-501 and 15/M503 shows a curb on top of a “concrete structural support” is the detail showing a new curb on existing concrete deck, or is it showing a new structural concrete slab on top of the existing concrete roof deck?
Answer: Provide new equipment curb on existing concrete roof deck.
1. Question: Please provide a specification section for roofing repairs related to the exhaust fan eqp pads shown on sheet MH104 and MH110. What is the existing roof type? What is the composition of the deck structure? Is the current roof under warranty? If so, please provide contact information for the vendor that holds the warranty.
Answer: Roof work is not required for this project. Should the contractor damage the roof, repairs shall be performed in accordance with the VA master specification 07 01 50.19. The roof warranty contact is Tremco 800-422-1195; warranty number 123318.
1. Question: Will the temp AHU feeders be left permanent, or removed after the temp unit is relocated/ removed?
Answer: All temporary ducts shall be removed by project completion.
1. Question: Note 22 on sheet E411 seems to be missing information. All boom motors are to be connected to…..?
Answer: This note is an artifact from a previous design option. The boom headwalls, as currently designed, do not include motors, and this keynote can be disregarded.
1. Question: Note 10 on sheet T-601 lists an articulating arm medical services boom “by others”. Please confirm if the medical booms are furnished or installed by the VA, or Furnished or installed by the contractor.
Answer: Contractor shall furnish and install the Medical service boom.
1. Question: If asbestos containing materials are encountered, please confirm that testing/remediation will be treated as a modification to the contract.
Answer: Confirmed.
1. Question: Please provide an equipment schedule for this project.
Answer: Equipment schedules for each discipline (mechanical, electrical and plumbing) are in the contract drawings.
1. Question: Will the temp AHU require a curb to be temporarily installed while in use, or will it be allowed to sit on dunnage on top of the existing roof?
Answer: No exceptions, curb is not required for temp unit.
1. Question: Please clarify if a superintendent can be a dual hat to cover the requirements of SSHO?
Answer: VA has no objection if the Superintendent is able to perform dual functions and is qualified per the specifications.
1. Question: If no to the question above, will the SSHO need to be full-time for this project?
Answer:
Full-time Safety officer is not needed on the job site; Superintendent can fill in this role if they qualify per the specifications.
1. Question: Please clarify if a superintendent can be a dual hat to cover the requirements of QA/QC?
Answer: Full-time QC technician is not needed at the job site. It is up to the Contractor to complete QA/QC checks and submit documentation on time for VA approval.
1. Question: If no to the question above, will the QA/QC need to be full time for this project?
Answer: Full-time QC technician is not needed at the job site. It is up to the Contractor to complete QA/QC checks and submit documentation on time for VA approval.
1. Question: Can a superintendent be a triple hat to cover the requirements of SSHO and QA/QC?
Answer: Performing all three job duties is permissible provided the Superintendent meets the job qualifications and experience in those fields. Contractor shall submit resume of the Superintendent for VA review and approval.
1. Question: Please confirm the issued drawing set is the latest and complete set.
Answer: Confirmed.
1. Question: Please confirm the current design documents meet current VA TIL guidelines.
Answer: Confirmed.
1. Question: Past Performance: If we have a PPQ, requested for another opportunity, that is less than 12 months old, can we use it within our proposal? Or will a new PPQ need to be requested?
Answer: PPQs are not required to be submitted with your proposal. Should you choose to submit one, the page(s) will count towards your page limit (reference proposal instructions on page 7 of the solicitation).
1. Question: Is the use of temporary plastic wall panels (such as STARC panels) acceptable?
Answer: Yes.
1. Question: Who is responsible for the terminal clean of the project area?
Answer: Contractor shall perform thorough cleaning and sterilize the area before final handover. VA EMS is responsible for terminal cleaning to ensure the area meets VA standards.
1. Question: Who is responsible for contracting the commissioning agent?
Answer: Contractor is responsible for full job completion including post TAB, final hook-up and commissioning, training the VA staff, and providing operating and maintenance instructions for new equipment.
1. Question: Please clarify the location of the laydown yard.
Answer: VA can allocate laydown space near the ER entrance, a small area in the tunnel, and if required, additional space is available in the grassy area near parking lot A.
1. Question: Please provide the POC for the patient lift manufacturer.

1. Question: Please clarify if the GC is responsible for the storage of the existing patient lifts.
Answer: Yes, GC is responsible for safe removal and storage of the existing patient lifts. See RFI #2’s response for storage location.
1. Question: Do the window shades need to be removed, replaced, or amended or simply repaired if they're damaged?
Answer: Window shades are to remain and be protected during construction. If the shades need to be removed in order to facilitate construction, the contractor is to do so without damaging the window shades or wall. Any damage will required patch/repair work at the contractor’s expense.
1. Question: Who is responsible for the removal of patient lifts prior to renovation? If the awarded contractor is responsible is there a place to store them on premises or is the contractor responsible for providing storage?
Answer: See RFI Response #2.
1. Question: The Legend page shows a Patient Station with a built-in Bed Connector receptacle. The Rauland Responder 5 Patient Station does not have that receptacle. To add a bed connection, we would need an additional single gang back box. Please confirm if we are required to add a backbox for the patient station.
Answer: Existing bed connection is existing to remain. No need for an additional backbox to be installed.
1. Question: Please confirm if the nurse call demo scope is for the removal of the existing Patient Station and cable.
Answer: The existing patient service wall shall be removed. Existing ports in the service wall shall be migrated into the boom.
1. Question: Although it isn’t specifically called out, can we assume that we are to reuse the Patient Stations and re-install them into a new “Boom” Arm shown on the plans?
Answer: Existing data, med gas, and electrical outlets shall be routed through the boom arm once the new boom arm is installed.
1. Question: On the Snip (T-101) below, we also see two TV symbols and we’re not clear on what happens there.
Answer: The TV and D outlets within the rooms are existing to remain.
1. Question: This floor has an existing Television and Connections at the location marked with the “2”. Do we assume these are “Existing to Remain?”
Answer: Correct, these are existing to remain.
