About this file

This is a Price Proposal Form (Exhibit II) for Contract No. 524048, an Agency Wide Paving contract administered by the Maryland Port Administration for work at all marine terminals in Baltimore, Maryland. The contract encompasses a comprehensive range of paving and related services including asphalt and concrete pavement removal, milling, patching, joint and crack sealing, bituminous overlays, slurry seal coating, pavement markings, utility adjustments, curb and gutter work, sidewalk installation, trenching, excavation, grading, sediment and erosion control, and traffic maintenance. The scope is organized into eight categories with 289 line items covering quantities ranging from 5 units to 260,000 square yards depending on the specific service. Line items include fixed allowances for unanticipated work ($700,000), test pavements ($100,000), temporary barriers ($10,000), ponding-related work ($250,000), site-specific health and safety plans ($25,000 and $20,000), chromium-contaminated materials handling ($40,000), and electrical work ($250,000), along with numerous unit-priced items for materials and labor.

Bidders must complete the Price Proposal Form exactly as provided without amendments, alterations, or blank items, with failure to comply resulting in a determination of non-responsiveness. The form requires submission of unit prices or lump sum amounts for all line items and includes a Maximum Percentage for Added or Changed Work (item MP-1) to be expressed numerically to one decimal place. The total proposed value across all items is $1,415,000.00, which represents the sum of all allowances and priced line items. The proposal must be signed in ink by an authorized representative—individual, partner, or corporate officer attested by the secretary—with documentation of authority required for corporate signatories. Contractors must declare their understanding that prices include all materials, labor, machinery, tools, and means necessary to complete the work as specified in the contract documents.
