About this file
This is a Vendor Questionnaire document for Broward County, Florida, related to a grills procurement solicitation for the County's Parks and Recreation Division multiple park locations. The questionnaire must be submitted with the solicitation response or within three business days of a written request from the County, with failure to timely submit potentially resulting in the vendor being deemed nonresponsive. The procurement is structured as a fixed contract requiring grills per specific County specifications, with purchase and delivery costs included in unit pricing, and all responses must be submitted electronically through the BPRO system by the specified closing date and time.
The questionnaire requires standard business information including legal name, contact details, Federal Employer ID, and authorized dealer/warranty confirmations, along with several compliance certifications. Key requirements include non-collusion certification, disclosure of any County employee relationships that could influence the procurement, confirmation of drug-free workplace programs for preference consideration when bids are tied, and acceptance of Visa credit card payments without additional fees. Vendors must also certify they have not participated in solicitation development that would provide unfair advantage and confirm they have not been debarred or suspended by any government entity within the past three years. The County reserves the right to request vendor references within three business days and may extend the same contract terms to other governmental entities during the contract period.
