About this file
This is a Trench Safety Act Requirement form for Broward County, Florida, related to the Judicial Court West Tower Repairs project. The form documents vendor compliance with Florida Trench Safety Act (Sections 553.60-553.64, Florida Statutes) and OSHA excavation safety standards (29 C.F.R. Section 1926.650), which apply to any project involving trenches exceeding 5 feet in depth. Vendors must complete and submit this form with their proposal or within three business days of the County's request; failure to comply with the submission timeframe may result in a nonresponsive determination. The form requires vendors to identify and document anticipated methods and costs for compliance with applicable trench safety standards, including any special shoring requirements if applicable.
The form provides two contract type options for vendors to complete: an open-end contract section requiring vendors to identify unit prices and methods for linear feet of work and square feet of special shoring, and a fixed contract section requiring vendors to specify quantities, unit prices, and methods. Vendors must provide detailed cost breakdowns and methodology descriptions for their trench safety compliance approach. All costs and methods identified by the vendor become legally binding on the vendor, its employees, and any subcontractors. The form was dated September 9, 2024, and requires vendor name and completion of all applicable sections prior to submittal.
