About this file

This is a Past Performance Reference Information Form template for RFQ No. AA-2026-05, issued by the University of California on behalf of Lawrence Berkeley National Laboratory (LBNL) under the Department of Energy. The form is limited to three pages per reference contract and must be submitted with all quote submissions regardless of contract value. Offerors are required to provide detailed information about their relevant past performance on similar contracts, including the reference client name and UEI, contract number, contract title, period of performance, contract type, total value, and complexity assessment that addresses work scope similarity, difficulty, and any problems encountered with corrective solutions implemented.

The form mandates comprehensive safety documentation, requiring offerors and their proposed lower-tier subcontractors to disclose safety records including Experience Modification Rate (EMR) data from Workers' Compensation insurance carriers for the past three years, with any EMR exceeding 1.0 requiring explanation. Additionally, offerors must fully disclose all OSHA, Cal-OSHA, and state occupational safety and health citations received within the past three years, categorizing violations by type and current status. The form emphasizes that failure to disclose any safety violations will result in a non-responsive offer or default if discovered after award. The RFQ is for flexible cables to be utilized in the ALS-U project, with quotes due April 23, 2026, and a Rep Cert Form required only if the total quote exceeds $150,000.
