About this file
This is a Vendor Disclosure Statement form issued by the Colorado Department of Corrections for state contracting purposes, specifically related to solicitation number DQ 2026 for a July Meat and Cheese Request for Quote. The form requires prospective vendors to disclose information regarding where contract services, including any subcontracts, will be performed. Vendors must identify all counties and states in which work will occur and provide explanations if services are anticipated to be performed outside the United States or the State of Colorado, including justifications for why such out-of-state or international performance is necessary or advantageous.
The disclosure statement applies to most state contracts but contains specific exemptions for Medicare, Colorado Medical Assistance Act programs, Children Basic Health Plan Act programs, Colorado Indigent Care Program contracts, and construction contracts. Vendors must complete the form by providing their name, solicitation number, service locations, and signatures with printed names and titles prior to contract award. This mandatory disclosure requirement ensures state visibility into vendor operations and supply chain activities before entering into contractual agreements.
