About this file
This is a Vendor Disqualification Checklist for Colorado's July Meat and Cheese Request for Quote. The checklist outlines mandatory documentation requirements that all vendors must submit with their bid or quote submissions through bidnetdirect.com. Required documents include Exhibit B (Vendor Information Form), which must be signed in ink or electronically; Exhibit C (W-9 Request for Taxpayer Identification Number); Exhibit D (Price Proposal); and Exhibit E (Vendor Disclosure Statement). All quotes must reference the appropriate DQ number and title and include all specified attachments to be considered eligible for evaluation.
The checklist serves as a preliminary screening tool to ensure vendor compliance with submission requirements prior to formal bid evaluation. Vendors are responsible for reviewing all remaining exhibits and attachments associated with the solicitation to ensure complete understanding of specifications and requirements. Failure to submit any required documentation or to follow submission procedures through the designated bidnetdirect.com portal may result in quote rejection.
