About this file

The document is a Vendor Disclosure Statement from the Colorado Department of Corrections for Solicitation Number DQ 2025000366, which requires prospective vendors to disclose information about contract performance locations. This form is specifically designed to capture details about where services will be performed, with particular emphasis on work conducted outside of the United States or Colorado. The form is mandatory for most state contracts, with explicit exceptions noted for certain medical and healthcare-related contracts and construction projects.

The disclosure statement requires vendors to specify the counties and states where contract services will be performed and provide justification if any work is anticipated to occur outside of the United States or Colorado. Vendors must complete and sign the form, detailing their company name, service locations, and reasoning for any out-of-state or out-of-country work. This document appears to be part of the procurement process for the Avigilon Cameras and Accessories contract, which aims to update and replace camera systems at multiple Colorado Department of Corrections facilities.
