About this file

This is a Vendor Disclosure and Notification form required by the Colorado Department of Corrections (CDOC) for non-construction contracts under Colorado Revised Statutes 24-102-206. The form is designed to be completed and submitted by vendors as part of their bid response and serves as official notice to the Department of Personnel when contract performance is anticipated outside the United States or the State of Colorado. Vendors must answer whether services will be performed outside these jurisdictions and, if so, must provide detailed information regarding the type of services, specific locations where work will be performed, and justification for performing work outside the US or Colorado. The form requires signature by an authorized representative, including the business President, Board Chairperson, or Contract's Authorized Signatory, along with printed name, title, and date.

The form includes procurement-use-only sections to be completed by the contracting agency, including the solicitation number (DQ 2027000102), contract type (Purchase Order), and verification that the vendor provided required notice within 20 days of contracting or deciding to contract outside the US or Colorado. This disclosure requirement applies to all non-construction service contracts and represents a mandatory compliance element for bid submission. The form appears twice in the document, suggesting it may be a template with both blank and example versions or revisions made during the solicitation process.
