About this file

This is a Vendor Disclosure and Notification form issued by the Colorado Department of Corrections (CDOC) for non-construction contracts under solicitation number DQ 2027000074. The form serves as notice to the Department of Personnel regarding contracts awarded to vendors who will perform services outside the United States or the State of Colorado, as required by Colorado Revised Statutes 24-102-206. Vendors must complete and return this document with their bid submission, with the form to be completed by the contractor's President, Board Chairperson, or Authorized Signatory. The form requires vendors to disclose whether any services or subcontract work will be performed outside the United States or Colorado, and if so, to describe the type of services, identify the specific countries or states where services will be performed, and explain the necessity or advantage of performing work outside these geographic boundaries.

The form includes a procurement tracking section requiring documentation of whether the vendor provided notice within 20 days of contracting or deciding to contract outside the US or Colorado. No specific pricing terms, set-asides, incumbent information, or funding amounts are detailed in this disclosure document, as the form functions solely as a compliance and notification mechanism for the state to monitor contract performance locations and to ensure vendors meet statutory notification requirements prior to award.
