About this file

This is a Vendor Disclosure & Notification form issued by the Colorado Department of Corrections (CDOC) for a Boiler Chemicals & Treatment Services contract opportunity in Colorado. The form serves as notice in accordance with Colorado Revised Statutes 24-102-206 and requires vendors to disclose whether any services under the contract or subcontracts will be performed outside the United States or State of Colorado. Vendors must complete and return this document with their bid submission and indicate whether services will be performed outside these jurisdictions. If services are to be performed outside the U.S. or Colorado, the vendor must specify the type of services, identify the country(ies) and/or state(s) where work will occur, and explain the necessity or advantage of performing services outside these locations. The form requires signature from the business President, Board Chairperson, or authorized signatory and submission to the sole contact listed in the solicitation (Solicitation Number: DQ 2027000071).

The form establishes a requirement that vendors provide notice within 20 days of contracting or deciding to contract outside the U.S. or Colorado. This disclosure mechanism ensures transparency regarding the geographic location of service delivery and allows the State of Colorado to monitor compliance with statutory requirements regarding out-of-state and out-of-country contract performance. The form functions as a mandatory compliance document that must accompany all bid submissions for this purchase order type contract.
