About this file

This document is a vendor disclosure and notification form (Exhibit E) issued by the Colorado Department of Corrections for non-construction contract performance outside the United States or Colorado. The form requires contractors bidding on the Foam Mattress Cores procurement (Solicitation Number DQ 2027000046) to disclose whether any services or subcontracted work will be performed outside U.S. or Colorado borders. Bidders must indicate whether contract performance will occur outside the specified geographic area and, if so, provide detailed information regarding the type of services to be performed, the specific countries or states where work will occur, and the justification for performing services outside the United States or Colorado. The form must be completed and returned with the bid submission to the sole contact listed in the solicitation.

The form serves as official notice to the Department of Personnel in compliance with Colorado Revised Statutes 24-102-206 and 24-102-205. A vendor or its authorized signatory must certify the information provided and submit the completed form as part of the bid package. The procurement office will document whether the vendor provided timely notice within 20 days of contracting or deciding to contract outside the U.S. or Colorado, as required by state statute. This disclosure requirement ensures transparency regarding offshore or out-of-state performance on the purchase order contract for foam mattress cores.
