About this file
This is a vendor disclosure and notification form issued by the Colorado Department of Corrections (CDOC) for the State of Colorado, required for non-construction contracts where services may be performed outside the United States or Colorado. The form requires bidders to disclose whether any contract services or subcontracted services will be performed outside U.S. or Colorado borders, and if so, to specify the type of services, the geographic location(s) where services will be performed, and the business justification for performing work outside these jurisdictions. The form must be completed and returned with the bid submission to the sole contact listed in the solicitation.
The document serves as official notice to the Colorado Department of Personnel regarding vendor performance of contract services outside established geographic boundaries. Completion of this form is mandatory for all bid submissions, with vendors required to provide either a "Yes" or "No" response to the primary disclosure question. For affirmative responses, vendors must provide detailed explanations of service types, specific countries or states involved, and rationale for external performance. The procurement office section requires documentation of whether the vendor provided required notice within 20 days of deciding to contract services outside the U.S. or Colorado, ensuring compliance with Colorado Revised Statutes 24-102-205 and 24-102-206.
