About this file

This is a vendor disclosure and notification form issued by the Colorado Department of Corrections (CDOC) for non-construction contracts requiring bidders to disclose any services to be performed outside the United States or Colorado. The form must be completed and returned with the bid submission to the sole contact listed in the solicitation. Bidders are required to answer whether any services under the contract or subcontracts will be performed outside the United States or Colorado, and if the answer is affirmative, they must describe the type of services, identify the specific countries and/or states where services will be performed, and explain the necessity or advantage of performing work outside these geographic boundaries. The form requires signature from an authorized representative such as the business president, board chairperson, or contract's authorized signatory, along with printed name, title, and date.

The procurement section of the form captures administrative details for internal use, including the solicitation number, contract type, services performed outside the U.S. or Colorado, and verification of whether the vendor provided notice within 20 days of contracting or deciding to contract outside the specified geographic area. This disclosure requirement is mandated by Colorado Revised Statutes 24-102-206 and 24-102-205, establishing a formal notification process to the Department of Personnel regarding any offshoring of contract work.
