About this file
This is a Vendor Disclosure and Notification form issued by the Colorado Department of Corrections (CDOC) for the State of Colorado, requiring completion and submission by bidders as part of the procurement process. The form mandates that vendors disclose whether any services under the contract or subcontracts will be performed outside the United States or the State of Colorado. Bidders must answer a yes or no question regarding offshore or out-of-state service delivery, and if services will be performed outside these jurisdictions, vendors must specify the types of services, the countries or states where services will be performed, and justify why such external performance is necessary or advantageous. The form requires authorization by the business President, Board Chairperson, or authorized contract signatory.
The form is designed to ensure compliance with Colorado Revised Statutes 24-102-206 and 24-102-205, which govern vendor notification requirements for out-of-state and offshore contracting. Vendors must provide written notice of any anticipated performance outside the United States or Colorado, and procurement staff must verify whether the vendor provided such notice within twenty days of contracting or deciding to contract outside the specified jurisdictions. This disclosure requirement applies to non-construction service contracts and serves as an administrative control mechanism for the state to track and monitor vendor activities conducted beyond Colorado's borders.
